Comprehensive Excel Template for Data Collection via Chore Chart (Client View)

Purpose: This Excel template is specifically designed for Data Collection within structured household or care environments using a Chore Chart. It enables caregivers, supervisors, or administrators to systematically track daily, weekly, and monthly chore completion by individuals (e.g., clients in assisted living homes, special education students, or family members). The Client View style ensures data is presented clearly and accessibly for non-technical users such as clients and their families.

The template integrates automated tracking features with visual indicators to encourage accountability, improve routine adherence, and provide a longitudinal record of behavior patterns—critical for progress evaluation in therapy or support programs.

Sheet Names

  • Client Summary Dashboard: High-level overview of chore completion rates, trends, and overdue tasks.
  • Daily Chore Log: Main data entry sheet where each day’s chores are recorded by client.
  • Chore Master List: Reference sheet containing all possible chores with metadata (category, frequency, point value).
  • Client Profile & Settings: User-configurable settings including names, default time slots, reward thresholds.

Formulas Required

To ensure accurate Data Collection, the following formulas are embedded across sheets:

  • =VLOOKUP(Chore Name, Chore Master List!$A$2:$D$100, 3, FALSE) → Auto-populates “Category” based on chore.
  • =IF(Status="Completed", TODAY(), "") → Marks completion date automatically.
  • =COUNTIFS(Daily Chore Log!$B:$B, Client Name, Daily Chore Log!$E:$E, "Completed") → Counts completed chores per client on the Dashboard.
  • =AVERAGEIF(Daily Chore Log!$F:$F, Client Name, Daily Chore Log!$G:$G) → Calculates average motivation level per client.
  • =SUMIFS(Daily Chore Log!$H:$H, Daily Chore Log!$B:$B, Client Name) / COUNTIF(Daily Chore Log!$B:$B, Client Name) → Computes average time spent per task.

Conditional Formatting

To support the Client View, visual cues are applied using Conditional Formatting:

  • Status Column: Red background if “Not Started” (overdue), green for “Completed”, yellow for “In Progress”.
  • Motivation Level (1–5): Color scale from red (1) to green (5).
  • Time Taken: Orange highlight if over 30 minutes, indicating potential inefficiency or difficulty.
  • Daily Checklist: Streak indicators for consecutive days with completed chores.

User Instructions

  1. Navigate to the Client Profile & Settings sheet and enter all client names in the designated list.

  2. In the Daily Chore Log, select a date and choose a client from the dropdown.

  3. Select each chore from the “Chore Name” dropdown; related data (Category, Frequency) will auto-fill.

  4. Mark status as “Completed”, “In Progress”, or “Not Started” after observation.

  5. Enter time taken and rate motivation level using the 1–5 scale.

  6. Add notes if relevant (e.g., "Struggled with zipping jacket").

  7. Save the file regularly. The Dashboard will update in real-time as new entries are made.

Recommended Charts & Dashboards (Client View)

The Client Summary Dashboard includes:

  • Completion Rate Bar Chart: Shows % of chores completed per client over the past 7 days.
  • Trend Line Graph: Tracks average motivation level and time spent across weeks.
  • Gauge Chart: Displays current streak (e.g., “5-day streak!”).
  • Pie Chart: Breakdown of chore types completed (Personal Care, Household, etc.).

All visualizations dynamically update with new data entries and are designed for easy interpretation by non-experts—fulfilling the core goal of Client View.

This template ensures reliable Data Collection through structured input, automation, and visualization—transforming a simple chore chart into a powerful tool for behavioral tracking and client support.

*Note: Time Taken can be manually entered or auto-calculated using time-tracking macros (optional VBA add-on).
