Last Service Date Date (MM/DD/YYYY) When the last maintenance check was performed.
Next Scheduled Service Date (MM/DD/YYYY) Automatically calculated based on service frequency. Updated after each entry.
Service Frequency List (Dropdown) Options: "Daily", "Weekly", "Monthly", "Quarterly", "Biannual", "Annual". Used for auto-calculation of next service.
Status List (Dropdown) Current status: "In Service", "Under Maintenance", "Out of Order", "Pending Inspection".
Maintenance Type List (Dropdown) Types: Preventive, Corrective, Routine Check, Upgrade.
Technician Name Text Name of the person who performed the service.
Date Completed Date (MM/DD/YYYY) When the maintenance task was finished.
Description of Work Text (Multi-line) Detailed notes on the service performed, including findings and actions taken.
Cost of Service Currency ($0.00) Expenses incurred for labor, parts, or supplies.

Formulas Required

To ensure automation and real-time tracking, the following formulas are implemented:
  • Item ID Auto-Generation: In cell A2: =IF(A1="", "EQUIP-001", "EQUIP-"&TEXT(VALUE(MID(A1,6,3))+1),"000")). This auto-increments the ID for new entries.

Conditional Formatting

The template uses visual cues to enhance readability and highlight critical information:
  • Overdue Items: Cells in column G (Status) turn red if the next service date is past today.
  • Critical Assets: If "Status" is "Out of Order", the entire row turns bright red and bold font.
  • Aging Equipment: Rows where last service was more than 90 days ago are highlighted in yellow.
  • Cosmetic/High Cost Items: Cells in "Cost of Service" over $500 are formatted with a red background and bold text.

User Instructions

1. Open the template in Microsoft Excel (version 2016 or later recommended). 2. To add a new maintenance record, enter data into the next available row on the "Maintenance Log" sheet. 3. Use dropdowns in columns like Service Frequency, Status, and Maintenance Type for consistency. 4. The template automatically calculates the next service date based on frequency and updates status accordingly. 5. To print, go to the "Printable Report" sheet, which contains a condensed view of all active maintenance items, due dates, and statuses formatted for professional printing (landscape orientation). 6.
