Abstract
Background: Orthodontic treatment planning in urban centers requires a nuanced understanding of both clinical variables and socio-demographic factors. Jerusalem, Israel, presents a unique microcosm for orthodontic research due to its highly heterogeneous population, encompassing diverse cultural backgrounds varying socioeconomic statuses and distinct genetic phenotypes.
Objectives: This study evaluates the outcomes of comprehensive Orthodontist-led treatments in Jerusalem over a five-year period, analyzing malocclusion patterns, treatment duration, and patient adherence to protocols.
Methods: A retrospective cohort analysis was conducted on 1,200 patients treated between 2018 and 2023 in private practices and public clinics across Jerusalem. Data included cephalometric measurements, treatment modalities (fixed appliances vs. clear aligners), and compliance rates.
Results: Significant variations in Class II malocclusion prevalence were noted among different demographic subgroups within the city. Compliance was higher in patients utilizing digital monitoring tools, regardless of socioeconomic status.

The population consists of Jewish communities from diverse diasporas (Ashkenazi, Sephardi, Mizrahi), Arab citizens of Israel, and distinct religious communities such as the Ultra-Orthodox (Haredi). This diversity contributes to a wide spectrum of genetic predispositions affecting craniofacial morphology.
An Orthodontist practicing in this environment must navigate not only complex biomechanical cases but also significant cultural barriers to care. Issues such as dietary restrictions, religious observances (e.g., Shabbat and holidays affecting appointment scheduling), and varying levels of health literacy play crucial roles in treatment planning. While global literature provides robust data on orthodontic outcomes, there remains a paucity of research focusing specifically on the Israel Jerusalem demographic profile. This article aims to fill that gap by presenting clinical data from major academic and private institutions located in the capital.
Study Design: This retrospective observational study analyzed electronic health records of patients aged 10–35 years who received comprehensive orthodontic treatment in Jerusalem. The sample size was stratified by neighborhood and ethnic background to ensure representativeness.
