This document serves as the comprehensive final report for my clinical and community pharmacy internship conducted within the vibrant healthcare landscape of the United States Miami region. As a Doctor of Pharmacy (Pharm.D.) candidate, this internship was designed to bridge the gap between academic theory and practical application, specifically focusing on patient-centered care in a diverse urban setting. The primary objective was to refine clinical skills, enhance communication abilities with patients from varied cultural backgrounds, and understand the unique operational dynamics of pharmacy practice within Miami's highly competitive market. This report details the activities undertaken, challenges faced, skills acquired, and professional growth experienced during this critical phase of my training as a Pharmacist in United States Miami. The role of a modern pharmacist extends far beyond dispensing medication; it requires a deep understanding of pathophysiology, pharmacology, and the socio-economic factors influencing health outcomes. My internship was structured to expose me to these complexities within United States Miami, a city known for its demographic diversity and high prevalence of chronic diseases such as diabetes and hypertension. The core objectives of this internship included:

  • Gaining hands-on experience in medication therapy management (MTM) under the supervision of licensed preceptors.
  • Demonstrating proficiency in pharmaceutical calculations, dispensing accuracy, and regulatory compliance specific to Florida State Board of Pharmacy regulations.
  • <
  • Developing effective communication strategies for interacting with a multilingual patient population typical of Miami's demographics.
  • Analyzing workflow efficiencies in both community and hospital settings to improve patient safety outcomes.
The internship was divided into two main phases: a community pharmacy rotation at a large retail chain in the heart of Miami Dade County, followed by an acute care rotation at a major hospital system in United States Miami. A. Community Pharmacy Rotation In the community setting, I worked primarily as a Pharmacist Intern supporting senior staff pharmacists. The volume of prescriptions processed was high, requiring meticulous attention to detail and time management. One of the most significant aspects of this rotation was dealing with insurance prior authorizations and formulary exceptions. I learned to navigate various pharmacy benefit manager (PBM) platforms effectively, ensuring that patients could access their medications without financial hardship. Furthermore, because United States Miami has a large Hispanic and Caribbean population, I frequently assisted patients in Spanish or Haitian Creole regarding medication adherence and side effects, highlighting the importance of cultural competency in pharmacy practice. B. Acute Care/Hospital Rotation The hospital rotation provided exposure to interdisciplinary teamwork. As a Pharmacist Intern here, I participated in rounding rounds with physicians and nurses for patients on the medical-surgical floors. This experience allowed me to recommend appropriate antibiotic dosages based on renal function, manage anticoagulation therapy for patients with atrial fibrillation, and review total parenteral nutrition (TPN) orders. The fast-paced environment of United States Miami hospitals taught me how to prioritize tasks during emergencies and maintain clear documentation in electronic health records (EHR). Working as a Pharmacist Intern in United States Miami presented several distinct challenges. Firstly, the sheer density of the patient population meant that walk-in traffic could be overwhelming. Learning to balance accuracy with efficiency was a steep learning curve. Secondly, regulatory compliance in Florida is stringent regarding controlled substances (Schedule II-V drugs). I had to rigorously adhere to record-keeping protocols and verification steps mandated by both federal DEA guidelines and state laws in United States Miami jurisdictions. Finally, managing difficult patient interactions required emotional intelligence and patience, particularly when addressing complaints about wait times or copay costs. By the conclusion of this internship, I have significantly enhanced my technical and soft skills:
  • Clinical Reasoning: Ability to identify drug-drug interactions and suggest alternative therapies.
  • Patient Counseling: Improved ability to explain complex medical instructions in simple, understandable language.
  • : Deepened understanding of HIPAA privacy laws and Florida-specific pharmacy statutes.
This internship has been an instrumental part of my journey toward becoming a licensed Pharmacist in the United States. The experience gained in United States Miami has provided me with a robust foundation in both clinical and operational aspects of pharmacy practice. The diversity of the patient population challenged me to become more adaptable and culturally aware, while the rigorous academic supervision ensured that my clinical judgments remain evidence-based. I am confident that these experiences have prepared me to contribute effectively to public health initiatives and provide high-quality pharmaceutical care in future professional roles. I would like to extend my sincere gratitude to my preceptors at both the community retail pharmacy and the acute care hospital for their mentorship. Special thanks are also due to the administration of United States Miami healthcare facilities for allowing me this opportunity, and to my faculty advisors at the University of Florida for their continuous guidance throughout this internship period.
