Invoice Notes & Chemist Certification
This Invoice has been prepared and authorized by a licensed Chemist registered with the Ethiopian Food and Drug Authority (EFDA) and the Addis Ababa City Health Bureau. All pharmaceutical products listed above have been sourced through approved supply chains within Ethiopia Addis Ababa and comply with national pharmacopeia standards.
The Chemist responsible for quality assurance on this order is Mr. Yohannes Girma (License No: CHM-ET-2019-0334). All cold-chain items must be stored at 2°C–8°C upon receipt. The customer is advised to inspect all Chemist-supplied goods within 48 hours of delivery and report any discrepancies to the billing department.
This Invoice is valid for payment within 15 calendar days from the date of issue. Late payments will incur a penalty of 2% per month as stipulated under Ethiopian commercial law. Payment may be made via bank transfer to CBE Account No. 1001-2345-6789 or through the National Bank of Ethiopia wire transfer facility in Addis Ababa.
Terms & Conditions – Chemist Invoice, Ethiopia Addis Ababa
- This Invoice constitutes a binding commercial agreement between Addis Chemist & Pharmaceutical Supplies PLC (hereinafter "the Chemist Supplier") and the customer as identified above, governed by the laws of the Federal Democratic Republic of Ethiopia.
- All products supplied under this Invoice are subject to the quality control protocols of the Ethiopian Food and Drug Authority. The Chemist Supplier warrants that all pharmaceutical and chemical items are genuine, within their expiry dates, and stored under appropriate conditions throughout the supply chain in Ethiopia Addis Ababa.
- The customer acknowledges that this Invoice includes both tangible pharmaceutical goods and professional Chemist services. The professional services component (Item 6) is non-refundable once the consultation period has commenced.
- Delivery is scheduled within 5 business days from the date of this Invoice. The Chemist Supplier shall coordinate with the customer's logistics team in Addis Ababa to confirm the exact delivery date and time slot. Force majeure events, including but not limited to road closures or regulatory holds by EFDA, may extend delivery timelines.
- Any disputes arising from this Invoice shall be resolved through the Addis Ababa City Commercial Court or through arbitration under the Ethiopian Arbitration and Conciliation Proclamation No. 1221/2020.
- The Chemist Supplier reserves the right to substitute equivalent products of equal or higher quality should any item become temporarily unavailable, provided prior written notice is given to the customer at least 48 hours before the scheduled delivery.
- This Invoice must be retained by the customer for a minimum of five (5) years in compliance with the Ethiopian Tax Administration Proclamation No. 979/2016. A duplicate copy may be requested by referencing Invoice No. INV-2025-04-01872.
