Invoice Notes & Welder Service Details
This Invoice covers the complete welding services rendered by our certified Welder team at the Philippines Manila project site. All welding operations were performed in strict accordance with AWS D1.1 Structural Welding Code and Philippine National Standard (PNS) 122:2019 for Welding of Steel Structures. The assigned Welder, Master Welder Juan Dela Cruz, holds a valid DOST-registered certification and has over 15 years of experience in structural and industrial welding within the Philippines Manila metropolitan area.
All weld joints were subjected to Non-Destructive Testing (NDT) including Radiographic Testing (RT) and Ultrasonic Testing (UT) to ensure full compliance with the project specifications. The Welder team operated under the supervision of a Level II NDT Inspector to guarantee that every weld meets the required quality standards for the Philippines Manila construction industry.
Payment for this Invoice is due within thirty (30) days from the date of issuance. Please reference Invoice No. MPW-2025-00487 in all correspondence and remittances. This Invoice is valid for the Philippines Manila jurisdiction and is subject to the tax regulations of the Bureau of Internal Revenue (BIR) of the Philippines.
Terms & Conditions
1. This Invoice is payable via bank transfer to BDO Unibank, Account No. 1234-5678-9012, in the name of Manila Precision Welding & Fabrication Co., Philippines Manila.
2. A late payment penalty of 1.5% per month shall be applied to any amount not settled by the due date stated on this Invoice.
3. All Welder services described in this Invoice are covered under a 12-month workmanship warranty from the date of final acceptance by the client.
4. Any disputes arising from this Invoice shall be resolved under the laws of the Republic of the Philippines, with jurisdiction in the courts of Philippines Manila.
5. This Invoice must be accompanied by a valid Official Receipt (OR) upon payment. The OR will be issued in compliance with BIR regulations for the Philippines Manila region.
6. The Welder services are subject to site safety protocols as mandated by the Department of Labor and Employment (DOLE) of the Philippines. All personnel operating in the Philippines Manila site must comply with OSH standards.
