In the bustling metropolis of Karachi, Pakistan, the demand for specialized eye care services has skyrocketed due to rapid urbanization and demographic changes. As a port city with a population exceeding 16 million people, Karachi faces unique challenges regarding public health infrastructure. While ophthalmologists are widely recognized in medical circles, the specific role of the Optometrist remains underappreciated by the general public and even within parts of the medical community in Pakistan.
This poster presentation aims to highlight how trained Optometrists serve as crucial primary eye care providers in Karachi. By shifting certain diagnostic and management responsibilities to optometric professionals, we can alleviate the burden on ophthalmologists, reduce wait times for patients, and ensure that visual health issues are detected earlier. This document explores the current landscape of optometry in Pakistan's largest city and proposes a framework for integrated care models.

This study utilized a mixed-methods approach conducted across various districts of Karachi, including Gulshan-e-Iqbal, Saddar, and Korangi. The data collection period spanned six months from January to June 2023.
Quantitative Phase: A cross-sectional survey was administered to 1,200 participants visiting private optometry clinics and public health camps in Karachi. Participants underwent comprehensive eye examinations, including visual acuity testing, refraction tonometry and retinal imaging where available.
Qualitative Phase: Semi-structured interviews were conducted with 30 licensed Optometrists practicing in Karachi and 20 ophthalmologists to understand interprofessional dynamics, referral patterns, and perceptions of the optometric profession in Pakistan.
High Prevalence of Refractive Errors: Our data indicates that approximately 65% of school-aged children in Karachi suffer from uncorrected refractive errors. These conditions directly impact academic performance but remain untreated due to lack access affordable corrective lenses and professional optometric evaluation.
Detection of Systemic Diseases: Optometrists in our study sample detected early signs of diabetic retinopathy in 12% of patients who were unaware they had diabetes. This highlights the critical role Optometrists play as gatekeepers for systemic health screening, particularly given the rising prevalence of diabetes in Pakistan.
Perception Gap: Interview data revealed a significant gap in public awareness. Many residents of Karachi still view eye care exclusively through the lens of ophthalmic surgery rather than preventive visual maintenance. However, patients who received comprehensive care from Optometrists reported higher satisfaction rates due to longer consultation times and thorough explanations.
The findings underscore the untapped potential of Optometrists in improving public health outcomes in Karachi. Unlike general practitioners who may lack specialized training, Optometrists are equipped with advanced diagnostic tools and clinical skills specific to visual system disorders. In a resource-limited setting like Pakistan, leveraging this expertise can significantly enhance healthcare efficiency.
However, regulatory hurdles persist. The legal framework in Pakistan currently restricts the ability of Optometrists to manage certain ocular pathologies independently. Furthermore, insurance coverage for optometric services is minimal, which limits access for lower-income populations in urban slums and peripheral areas of Karachi.
This poster presentation asserts that Optometrists are indispensable assets to the healthcare ecosystem in Pakistan, particularly in densely populated urban centers like Karachi. By integrating Optometry more formally into the public health strategy, we can achieve earlier detection of eye diseases, improve educational outcomes for children through better vision care, and provide cost-effective solutions for systemic disease screening.
We call upon policymakers hospital administrators and medical associations in Karachi to advocate for expanded scopes of practice for Optometrists. Collaborative models between optometry clinics ophthalmology departments can ensure that the visual health needs of Karachi's diverse population are met comprehensively and equitably.
