1. Scope of Purchase OrderThis Purchase Order is issued by the Addis Ababa City Health Bureau to procure the professional services of a certified Dietitian for the implementation of community nutrition programs, clinical dietary counseling, and institutional meal planning across selected health facilities in Ethiopia Addis Ababa. The Dietitian engaged under this Purchase Order shall provide comprehensive nutritional assessment, therapeutic diet formulation, and ongoing dietary education services in alignment with the Federal Ministry of Health guidelines for Ethiopia Addis Ababa public health initiatives.

3.1 This Purchase Order constitutes a binding agreement between the Addis Ababa City Health Bureau and GreenLeaf Nutrition & Dietetics Consultancy PLC for the provision of Dietitian services as specified herein. All services shall be rendered within the administrative boundaries of Ethiopia Addis Ababa unless otherwise authorized in writing by the Purchasing Entity.
3.2 The Dietitian assigned under this Purchase Order must hold a valid license issued by the Ethiopian Food and Drug Authority (EFDA) and possess a minimum of five (5) years of post-graduate experience in clinical and community nutrition. The Dietitian must be a registered member of the Ethiopian Association of Dietitians and Nutritionists.
3.3 Payment terms: The total amount of ETB 1,080,000.00 (One Million and Eighty Thousand Ethiopian Birr) shall be disbursed in three (3) equal installments of ETB 360,000.00 each, payable within thirty (30) calendar days following the submission and approval of quarterly performance reports by the Dietitian to the Addis Ababa City Health Bureau.
3.4 The Supplier shall provide a comprehensive Dietitian service report at the end of each quarter, detailing patient outcomes, community program reach, and nutritional indicators for the Ethiopia Addis Ababa service area. Failure to submit reports within the stipulated timeframe shall result in a penalty of 2% of the quarterly payment per day of delay.
3.5 All services under this Purchase Order shall comply with the Ethiopian Public Procurement Proclamation No. 1160/2019 and the implementing regulations of the Federal Democratic Republic of Ethiopia. The Dietitian services must adhere to the Ethiopian Dietary Guidelines and the National Nutrition Policy of Ethiopia Addis Ababa.
3.6 The Supplier warrants that the Dietitian engaged shall maintain professional indemnity insurance with a minimum coverage of ETB 5,000,000.00 throughout the duration of this Purchase Order. Proof of insurance shall be submitted prior to the commencement of services.
3.7 Either party may terminate this Purchase Order with a written notice of thirty (30) days. In the event of termination, the Supplier shall be compensated for services rendered up to the date of termination, subject to final audit by the Ethiopia Addis Ababa City Health Bureau finance department.
3.8 All disputes arising from this Purchase Order shall be resolved through amicable negotiation. If unresolved within sixty (60) days, the matter shall be referred to the competent courts of Ethiopia Addis Ababa in accordance with Ethiopian law.
NOTE: This Purchase Order is subject to the availability of budget allocation from the Ethiopia Addis Ababa City Health Bureau FY 2025/2026 operational budget. The Dietitian services described herein are non-transferable and must be personally performed by the licensed Dietitian named in the contract annexure. Substitution of the Dietitian requires prior written approval from the Purchasing Entity. 4. Delivery and Performance ScheduleThe Dietitian services shall commence on 01 July 2025 and shall continue for a period of twelve (12) months, concluding on 30 June 2026. All service delivery locations are within the administrative jurisdiction of Ethiopia Addis Ababa, specifically at Bole Comprehensive Specialized Hospital, selected Woreda health centers in Bole, Kirkos, and Arada sub-cities, and designated government office canteens. The Dietitian shall maintain a minimum of five (5) working days per week at the primary clinical site and shall be available for emergency nutritional consultations as directed by the Ethiopia Addis Ababa City Health Bureau.
