SCOPE OF SERVICES – DIETITIAN CONSULTATION & NUTRITION PROGRAMMEThis Purchase Order is issued by HealthBridge Wellness Pvt. Ltd., India Mumbai, to engage the services of a certified Dietitian for the comprehensive nutritional management and wellness programme of 250 employees across our India Mumbai office locations. The Dietitian shall provide all services in compliance with the Indian Dietetic Association (IDA) code of conduct and applicable regulations under the Ministry of Health and Family Welfare, Government of India.

- This Purchase Order constitutes a binding agreement between HealthBridge Wellness Pvt. Ltd. (India Mumbai) and the named Dietitian provider, NutriCare Mumbai, for the delivery of nutritional and dietary services as specified herein.
- The Dietitian shall maintain valid professional registration with the Indian Dietetic Association and shall carry professional indemnity insurance of not less than INR 50,00,000 for the duration of this Purchase Order.
- All services under this Purchase Order shall be rendered at the client's premises in India Mumbai unless otherwise agreed in writing. The Dietitian shall arrive at least 15 minutes prior to each scheduled session.
- Payment shall be made within 30 days of receipt of a valid tax invoice from the Dietitian provider. Invoices must reference this Purchase Order number (PO/HBW/2025/00472) and include the provider's GSTIN as registered in India Mumbai.
- The Dietitian shall maintain strict confidentiality of all employee health records in accordance with the Information Technology Act, 2000, and applicable data protection norms in India. No employee dietary data shall be shared with third parties without written consent.
- Any modification, cancellation, or extension of this Purchase Order must be documented in a written amendment signed by both parties. Verbal instructions shall not be binding.
- The Dietitian shall provide a comprehensive end-of-contract report summarising all interventions, employee progress metrics, and recommendations for the subsequent quarter. This report shall be submitted to the HR department at India Mumbai headquarters within 7 working days of contract completion.
- In the event of non-performance or breach by the Dietitian, HealthBridge Wellness Pvt. Ltd. reserves the right to terminate this Purchase Order with 15 days' written notice and withhold payment for undelivered services.
- This Purchase Order shall be governed by and construed in accordance with the laws of India, with jurisdiction vested in the courts of India Mumbai, Maharashtra.
- All dietary recommendations provided by the Dietitian are for wellness and preventive purposes and do not constitute medical treatment. The Dietitian shall refer any employee requiring clinical intervention to a registered medical practitioner.
