This Purchase Order is issued by Wellington Community Health Services Limited to procure the professional services of a Registered Dietitian to deliver evidence-based nutritional counselling, dietary assessment, and community health education programmes across the New Zealand Wellington region. The Dietitian shall operate in full compliance with the New Zealand Code of Ethics for Dietitians and Nutritionists, as well as all applicable local bylaws and health regulations governing the provision of dietary advice in New Zealand Wellington.
The Dietitian engaged under this Purchase Order shall provide services at the Wellington Community Health Centre (Lambton Quay), the Porirua Community Hub, and the Hutt Valley Outreach Clinic. All dietary plans, nutritional assessments, and patient education materials produced by the Dietitian must be tailored to the diverse cultural and dietary needs of the New Zealand Wellington population, including Māori and Pasifika communities, and must align with the New Zealand Dietary Guidelines published by the Ministry of Health.

- Compliance: The Dietitian shall maintain valid registration with the New Zealand Dietitians Association throughout the contract period and shall comply with all New Zealand health and safety legislation, including the Health and Safety at Work Act 2015, as applicable to the New Zealand Wellington region.
- Confidentiality: All patient dietary records, nutritional assessments, and personal health information handled by the Dietitian shall be protected in accordance with the New Zealand Privacy Act 2020. No patient data shall be stored outside the New Zealand Wellington region without explicit written consent.
- Performance Standards: The Dietitian shall deliver all services in a timely manner and in accordance with the clinical protocols established by Wellington Community Health Services Limited. Failure to meet agreed service levels for two consecutive months shall constitute a material breach of this Purchase Order.
- Insurance: The Dietitian and the supplier entity shall maintain professional indemnity insurance of no less than NZD 5,000,000 and public liability insurance of no less than NZD 10,000,000 for the duration of this Purchase Order.
- Termination: Either party may terminate this Purchase Order with 60 days' written notice. In the event of termination, the Dietitian shall complete all in-progress patient dietary plans and provide a full handover report to the New Zealand Wellington health team.
- Dispute Resolution: Any disputes arising under this Purchase Order shall first be attempted to be resolved through mediation in New Zealand Wellington. If unresolved, the matter shall be referred to the District Court of Wellington, New Zealand.
- Intellectual Property: All dietary programmes, meal plans, and educational materials developed specifically for the New Zealand Wellington community health programme under this Purchase Order shall remain the property of Wellington Community Health Services Limited.
- Amendments: No amendments to this Purchase Order shall be valid unless made in writing and signed by authorised representatives of both parties.
This Purchase Order is issued and authorised by the undersigned on behalf of Wellington Community Health Services Limited. The supplier acknowledges receipt of this Purchase Order and confirms acceptance of all terms, conditions, and the scope of Dietitian services outlined herein for delivery in New Zealand Wellington.
