Scope of Purchase Order — Dietitian ServicesThis Purchase Order is issued by Meridian Health & Wellness Group Pte. Ltd. to engage the services of a qualified Dietitian for the provision of comprehensive nutritional counselling, dietary assessment, and evidence-based meal planning services. The Dietitian shall render all professional services in accordance with the standards set by the Singapore Dietetic Association and in full compliance with the regulatory requirements applicable to healthcare professionals practising in Singapore Singapore. The scope of work under this Purchase Order encompasses both in-person consultations at the Buyer's facility located in Singapore Singapore and remote telehealth sessions as mutually agreed upon in writing.

1. Acceptance and Binding Effect: This Purchase Order shall become a binding contractual agreement upon written acceptance by the Dietitian or the Dietitian's authorised representative. By signing below, the Dietitian acknowledges and agrees to all terms, conditions, and service specifications outlined in this Purchase Order for the provision of professional dietary and nutritional services in Singapore Singapore.
2. Professional Qualifications: The Dietitian engaged under this Purchase Order must hold a valid registration with the Singapore Dietetic Association and maintain current professional indemnity insurance with a minimum coverage of SGD 1,000,000. The Dietitian shall provide proof of registration and insurance certificates prior to the commencement of services in Singapore Singapore.
3. Service Delivery Location: All in-person Dietitian consultations, workshops, and on-site assessments shall be conducted at the Buyer's designated facility in Singapore Singapore, unless otherwise agreed in writing. Remote services may be delivered via approved telehealth platforms compliant with the Personal Data Protection Act (PDPA) of Singapore Singapore.
4. Payment Terms: Payment for services rendered under this Purchase Order shall be made within thirty (30) calendar days from the date of receipt of a valid tax invoice. Payment shall be processed via bank transfer to the Dietitian's designated account. No payment shall be due for services not yet rendered or not approved in writing by the Buyer's Procurement Manager.
5. Confidentiality and Data Protection: The Dietitian shall treat all client health information, dietary records, and personal data obtained during the course of this Purchase Order as strictly confidential. All data handling must comply with the PDPA as enforced in Singapore Singapore. No client information shall be disclosed to third parties without explicit written consent from the data subject.
6. Performance Standards: The Dietitian shall adhere to the evidence-based clinical nutrition guidelines published by the Singapore Ministry of Health and the Singapore Dietetic Association. All dietary recommendations and meal plans must be culturally appropriate for the diverse workforce in Singapore Singapore and must account for local food availability and dietary preferences.
7. Cancellation and Termination: Either party may terminate this Purchase Order with thirty (30) days' written notice. 8. Dispute Resolution: Any disputes arising from this Purchase Order shall be resolved through mediation in Singapore Singapore in accordance with the rules of the Singapore Mediation Centre. If mediation fails, disputes shall be referred to arbitration under the Singapore International Arbitration Centre (SIAC) rules. 9. Insurance and Liability: The Dietitian shall maintain professional liability insurance throughout the duration of this Purchase Order. The Buyer shall not be held liable for any adverse outcomes arising from the Dietitian's professional negligence or failure to adhere to the scope of work defined herein.
