Description of Services – Dietitian Engagement

This Purchase Order is issued by East African Health & Nutrition Services Ltd. to engage the services of a qualified and registered Dietitian for the provision of comprehensive clinical nutrition and dietary counselling services. The Dietitian, Dr. Amina Mwangi, shall be responsible for conducting nutritional assessments, developing individualised meal plans, and providing ongoing dietary guidance to patients and staff within our facilities located in Tanzania Dar es Salaam. This Purchase Order governs the scope, duration, compensation, and professional obligations of the Dietitian engagement as outlined below.

Terms and Conditions

1. This Purchase Order constitutes a binding agreement between East African Health & Nutrition Services Ltd. and the named Dietitian upon signature by both parties. All services shall be rendered in accordance with the professional standards set by the Tanzania Medical and Dental Association (TMDA) and the Tanzania Food and Drugs Authority (TFDA).
2. The Dietitian shall maintain valid professional registration and malpractice insurance throughout the duration of this Purchase Order. Any lapse in registration shall result in immediate suspension of services without penalty to the ordering party.
3. All dietary assessments, meal plans, and patient records generated under this Purchase Order shall be stored in compliance with the Tanzania Data Protection Act, 2022, and shall remain the property of East African Health & Nutrition Services Ltd.
4. The Dietitian is required to conduct all in-person services at the designated facilities within Tanzania Dar es Salaam, specifically at the Kariakoo Medical Centre and the Masaki Corporate Wellness Suite. Travel within the city limits is included in the unit prices stated above.
5. Payment shall be processed within thirty (30) calendar days from the date of invoice submission. Invoices must reference this Purchase Order number (PO/EAHN/2025/0347) and include a valid Tax Invoice as required by the Tanzania Revenue Authority (TRA).
6. The Dietitian shall submit a written progress report at the end of each month detailing the number of patients assessed, meal plans issued, sessions conducted, and any notable clinical observations relevant to the nutrition programme in Tanzania Dar es Salaam.
7. Either party may terminate this Purchase Order with fourteen (14) days written notice. In the event of early termination, the Dietitian shall be compensated for all services rendered up to the termination date.
8. All disputes arising from this Purchase Order shall be resolved through mediation in Tanzania Dar es Salaam in accordance with the laws of the United Republic of Tanzania. The competent courts of Dar es Salaam shall have exclusive jurisdiction.
9. The Dietitian shall adhere to all health, safety, and infection control protocols in effect at the facilities. Failure to comply may result in immediate removal from the premises and termination of this Purchase Order.
10. This Purchase Order is valid for a period of three (3) months from the Required Delivery Date. Renewal shall be subject to mutual written agreement and a new Purchase Order issuance.
