This Purchase Order is issued to formally engage the services of a qualified Doctor General Practitioner to provide comprehensive primary healthcare services within the jurisdiction of Afghanistan Kabul. The Doctor General Practitioner shall be responsible for the diagnosis, treatment, and ongoing management of a broad spectrum of medical conditions affecting the general population in the Kabul metropolitan area and its surrounding districts.
The scope of services under this Purchase Order includes, but is not limited to: routine patient consultations, preventive health screenings, management of chronic diseases (including diabetes, hypertension, and respiratory conditions), minor surgical procedures, prescription of medications, referral coordination to specialist physicians, maternal and child health services, immunization programs, and emergency first-response care. The Doctor General Practitioner is expected to maintain a minimum of 40 patient consultations per working day and to be available for on-call emergency services during designated hours.

Payment shall be processed on a monthly basis via bank transfer to the account designated by the Doctor General Practitioner. Invoices must be submitted by the 5th of each month for the preceding month's services. Payment will be disbursed within fifteen (15) business days of invoice approval by the Afghanistan Kabul Regional Health Authority finance department.
5.1 The Doctor General Practitioner shall maintain valid medical licensure issued by the Afghanistan Medical Council throughout the entire duration of this Purchase Order. Any lapse in licensure shall constitute grounds for immediate termination of this Purchase Order.
5.2 The Doctor General Practitioner is required to adhere to the ethical and professional standards established by the Ministry of Public Health of Afghanistan and the World Health Organization guidelines applicable to primary healthcare delivery in Afghanistan Kabul.
5.3 All patient records, medical files, and health data generated under this Purchase Order shall be maintained in strict accordance with Afghan data protection regulations. Confidentiality of patient information is paramount and any breach shall result in disciplinary action and potential legal proceedings.
5.4 The Doctor General Practitioner shall submit a written monthly report to the Afghanistan Kabul Regional Health Authority detailing patient volumes, common diagnoses, treatment outcomes, medication utilization, and any public health concerns observed in the community.
5.5 Either party may terminate this Purchase Order with thirty (30) days written notice. In the event of termination for cause, the terminating party must provide a detailed written justification.
5.6 This Purchase Order shall be governed by the laws of the Islamic Republic of Afghanistan. Any disputes arising from this Purchase Order shall be resolved through the administrative courts of Kabul, Afghanistan.
By signing below, both parties acknowledge and agree to all terms, conditions, and obligations set forth in this Purchase Order for the engagement of a Doctor General Practitioner to deliver essential healthcare services in Afghanistan Kabul. This document constitutes a binding agreement between the parties upon execution.
