NOTE: This Purchase Order is issued specifically for the operational requirements of Dr. Haruki Tanaka, a Doctor General Practitioner registered with the Kyoto Prefectural Medical Board. All items listed herein are intended for use at the Kyoto Central General Practice Clinic located in Higashiyama-ku, Japan Kyoto. The Doctor General Practitioner has verified that all equipment meets the standards set forth by the Japan Medical Device and Materials Management Association (JMDMA) and complies with local Japan Kyoto municipal health regulations. Delivery must be completed to the Japan Kyoto address listed above no later than 28 June 2025. TERMS AND CONDITIONS

1. This Purchase Order constitutes a binding agreement between the Buyer (Kyoto Central General Practice Clinic, operated by the Doctor General Practitioner Dr. Haruki Tanaka) and the Supplier (MediSupply Kanto Co., Ltd.) upon acceptance and signature by both parties.

2. All goods shall be delivered to the specified Japan Kyoto address in Higashiyama-ku. The Supplier shall bear full responsibility for safe transport and shall notify the Buyer at least 48 hours prior to delivery to the Japan Kyoto clinic premises.

3. The Doctor General Practitioner reserves the right to inspect all delivered items upon arrival. Any defective, damaged, or non-conforming goods must be reported within five (5) business days of delivery. The Supplier shall replace or refund non-conforming items at no additional cost to the Buyer.

4. Payment shall be made via bank transfer to the Supplier's designated account within thirty (30) calendar days of the date of this Purchase Order, provided all items have been received and accepted by the Doctor General Practitioner.

5. All medical devices and equipment listed in this Purchase Order must carry valid Japanese Medical Device Approval (Kikaku) certification. The Supplier warrants that all items comply with the Pharmaceuticals and Medical Devices Act (PMDA) regulations applicable in Japan Kyoto and throughout the Kansai region.

6. The Supplier shall provide a comprehensive warranty of no less than two (2) years on all electronic and mechanical equipment. Consumable items are covered under a 90-day quality guarantee.

7. This Purchase Order is governed by the laws of Japan. Any disputes arising from this document shall be resolved through the Kyoto District Court in Japan Kyoto.

8. The Doctor General Practitioner authorizes the procurement of these items under the clinic's annual budget allocation for the fiscal year 2025, as approved by the Kyoto Prefectural Health and Welfare Department.
