All goods and services referenced in this Purchase Order shall be delivered to the premises of the Doctor General Practitioner at 1428 Finchley Road, Hampstead, United Kingdom London, NW3 6QL. Delivery must be scheduled between 08:00 and 16:00 on a weekday to minimise disruption to patient consultations. The supplier is required to coordinate with the practice manager, Mr. David Okafor, at least 48 hours prior to delivery. All equipment must be installed, calibrated, and tested on-site before the supplier's delivery personnel depart. The Doctor General Practitioner, Dr. Eleanor Whitmore, or her designated clinical lead, will sign the delivery note to confirm acceptance of all items listed in this Purchase Order.
- This Purchase Order is issued in accordance with NHS England procurement guidelines and the Public Contracts Regulations 2015 where applicable. All transactions are governed by the laws of England and Wales, United Kingdom London jurisdiction.
- The supplier warrants that all medical devices and consumables supplied are CE-marked or UKCA-marked and fully compliant with the UK Medical Devices Regulations 2002 (as amended). The Doctor General Practitioner reserves the right to reject any item that does not meet the specified clinical standards.
- Payment shall be made by BACS transfer within 30 calendar days of receipt of a valid tax invoice, provided that all goods and services have been delivered and accepted in accordance with this Purchase Order. Late payment shall attract interest at the rate of 4% per annum above the Bank of England base rate, in line with the Late Payment of Commercial Debts (Interest) Act 1998.
- The supplier shall maintain adequate public liability insurance (minimum £10,000,000) and professional indemnity insurance throughout the duration of this Purchase Order and for a period of two years following completion of all obligations.
- All personal data processed in connection with this Purchase Order shall be handled in strict compliance with the UK General Data Protection Regulation (UK GDPR) and the Data Protection Act 2018. No patient-identifiable information shall be included in any documentation.
- In the event of a dispute arising from this Purchase Order, both parties agree to attempt resolution through mediation before pursuing litigation in the courts of United Kingdom London. The supplier acknowledges that the Doctor General Practitioner operates under NHS contractual obligations and that any supply disruption may affect patient care.
- This Purchase Order may be amended only by written agreement signed by both parties. No verbal modifications shall be binding. The supplier must confirm acceptance of this Purchase Order in writing within five (5) working days of receipt.
- The supplier shall comply with all applicable health and safety legislation, including the Health and Safety at Work etc. Act 1974, during the delivery and installation of goods at the United Kingdom London premises.
