3 Laboratory Technician Onboarding, Training, and Compliance Package. Includes initial orientation to Pacific Northwest Diagnostics protocols, safety training per WHMIS 2015 standards, laboratory safety certification, and registration with the appropriate Canada Vancouver regulatory bodies. 4 Annual Laboratory Equipment Calibration and Maintenance Support. The assigned Laboratory Technician shall be responsible for scheduling and executing quarterly calibration of all analytical instruments, maintaining service logs, and coordinating with vendor technicians for major overhauls. Covers all equipment located at the Canada Vancouver laboratory site. 1 12 months $6,400.00 $6,400.00
5 Professional Liability Insurance and Workers' Compensation Coverage. Full coverage for the Laboratory Technician personnel as required under the Workplace Safety and Insurance Act (British Columbia) and applicable Canada Vancouver municipal bylaws.   1. This Purchase Order constitutes a binding agreement between Pacific Northwest Diagnostics Inc. (hereinafter "the Buyer") and Coastal Lab Staffing Solutions Ltd. (hereinafter "the Supplier") for the procurement of Laboratory Technician professional services in the Canada Vancouver metropolitan area.
  2. The Supplier shall ensure that all Laboratory Technician personnel assigned under this Purchase Order hold current and valid certifications recognized by the Canadian Association for Clinical Laboratory Science (CALS) and meet all competency requirements established by the British Columbia College of Technologists and Technicians.
  3. All Laboratory Technician services shall be performed at the Buyer's primary facility located in Canada Vancouver, specifically at 1200 West Georgia Street, Suite 850, Vancouver, BC. The Supplier is responsible for arranging transportation and any applicable parking permits for the Canada Vancouver site.
  4. The Laboratory Technician personnel shall adhere to all applicable federal and provincial regulations, including but not limited to the Canada Labour Code, the Occupational Health and Safety Act (British Columbia), the Personal Information Protection Act (PIPA), and all Health Canada laboratory accreditation standards (ISO 15189).
  5. Payment shall be made within thirty (30) calendar days of receipt of a valid invoice from the Supplier. Invoices must reference this Purchase Order number (PO-2025-VAN-LAB-0047) and include the Supplier's GST/HST registration number. Payment shall be made via electronic funds transfer (EFT) to the account designated by the Supplier.
  6. The Supplier warrants that all Laboratory Technician personnel shall be properly insured, including professional liability insurance and workers' compensation coverage as mandated in Canada Vancouver. Certificates of insurance shall be provided to the Buyer prior to the commencement of services.
  7. Either party may terminate this Purchase Order with thirty (30) days' written notice. In the event of termination, the Buyer shall be liable for all services rendered and expenses incurred up to the effective date of termination.
  8. All disputes arising from this Purchase Order shall be governed by the laws of the Province of British Columbia and the federal laws of Canada applicable therein. Any legal proceedings shall be conducted in the courts of Canada Vancouver, New Westminster, or Surrey.
  9. The Supplier shall maintain strict confidentiality regarding all patient data, proprietary laboratory methods, and trade secrets encountered during the performance of Laboratory Technician duties. A separate Non-Disclosure Agreement (NDA) is attached as Appendix A to this Purchase Order.
  10. This Purchase Order is valid for a period of twelve (12) months from the Required Delivery Date. Renewal shall be subject to mutual written agreement and a new Purchase Order issued by the Buyer.
