- This Purchase Order is issued by the Philippine Institute of Medical Sciences, Manila Research Division, and is binding upon acceptance by the supplier. All goods and services shall be delivered to the designated laboratory facilities located in Metro Manila, Philippines.
- The Medical Researcher named in this document, Dr. Elena M. Santos, shall serve as the primary point of contact for technical specifications, quality verification, and acceptance of all delivered items. No item shall be considered accepted until inspected and signed off by the designated Medical Researcher or her authorized delegate.
- All equipment and consumables must comply with the standards set by the Philippine Food and Drug Administration (FDA) and the Department of Health (DOH) of the Philippines. Certificates of Analysis and Material Safety Data Sheets (MSDS) must accompany each shipment.
- Delivery shall be made to the PIMS Manila Research Division premises at 1200 E. Rodriguez Jr. Avenue, Quezon City, Metro Manila, Philippines. The supplier is responsible for all transportation, insurance, and handling costs within the Philippines Manila metropolitan area until goods are received and inspected.
- Payment shall be processed within thirty (30) calendar days from the date of invoice submission, provided that all items have been inspected, accepted by the Medical Researcher, and all required government procurement documentation has been completed in accordance with Republic Act No. 9184 (Government Procurement Reform Act).
- The supplier warrants that all items are new, unused, and free from defects. A minimum warranty period of twelve (12) months from the date of acceptance applies to all capital equipment. Consumable items are warranted to meet the specifications stated in this Purchase Order.
- Any deviation from the specifications outlined in this Purchase Order must be approved in writing by the Medical Researcher prior to delivery. Unauthorized substitutions will result in rejection of the affected items at the supplier's expense.
- This Purchase Order is governed by the laws of the Republic of the Philippines. Any disputes arising from this transaction shall be resolved through the appropriate courts of competent jurisdiction in Manila, Philippines.
- The supplier acknowledges that the items procured under this Purchase Order are intended exclusively for medical research purposes conducted by the Medical Researcher and her team at the PIMS Manila facility. Misuse or diversion of materials is strictly prohibited.
- All personal data processed in connection with this Purchase Order shall be handled in accordance with the Philippine Data Privacy Act of 2012 (Republic Act No. 10173).
