This Purchase Order is issued by the Lagos Metropolitan Health Services Authority to procure the professional services of a qualified Nurse for deployment at the Lagos General Hospital, Yaba, Lagos, Nigeria. The Nurse shall be responsible for providing comprehensive patient care, administering prescribed medications, monitoring vital signs, assisting physicians during surgical and clinical procedures, maintaining accurate patient records, and coordinating with multidisciplinary healthcare teams to ensure the highest standard of medical care for all patients within the facility.
The Nurse engaged under this Purchase Order must hold a valid registration with the Nursing and Midwifery Council of Nigeria (NMCN), possess a minimum of five (5) years of post-qualification clinical experience in a tertiary healthcare facility in Lagos or another major Nigerian city, and demonstrate proficiency in both critical care and general ward nursing. The Nurse shall be required to work a rotational shift schedule as determined by the hospital's nursing management, including weekend and public holiday duties in accordance with the standard Lagos healthcare staffing framework.

4.1 This Purchase Order is governed by the laws of the Federal Republic of Nigeria and the regulations of Lagos State. All disputes arising from this Purchase Order shall be resolved through arbitration in Lagos, in accordance with the Arbitration and Conciliation Act, Cap A18, Laws of the Federation of Nigeria, 2004.
4.2 The Nurse engaged under this Purchase Order shall be subject to the code of professional conduct established by the Nursing and Midwifery Council of Nigeria. Any breach of professional ethics, negligence in patient care, or failure to comply with hospital protocols in Lagos shall constitute grounds for immediate termination of this Purchase Order without further liability to the ordering entity.
4.3 The supplier, Excellence Nursing & Allied Health Services Ltd., warrants that the Nurse named in this Purchase Order is fully qualified, licensed, and insured to practise nursing in Lagos, Nigeria. The supplier shall indemnify the ordering entity against any claims arising from the Nurse's unauthorised or negligent practice.
4.4 Payment under this Purchase Order shall be made via bank transfer to the supplier's designated account in a Nigerian commercial bank. No cash payments shall be accepted. All invoices must reference this Purchase Order number (NGA-LAG-2025-04782) to be processed.
4.5 The Nurse shall be required to reside within the Lagos State jurisdiction during the contract period to ensure prompt availability for emergency call-outs and to comply with the Lagos State Health Workforce Deployment Policy.
The Nurse shall report for duty at the Lagos General Hospital, Yaba, Lagos, Nigeria, no later than 08:00 hours on 01 July 2025. The supplier shall ensure that all onboarding documentation, including the Nurse's NMCN registration certificate, professional indemnity insurance, police clearance from the Nigeria Police Force (Lagos Command), and medical fitness certificate, are submitted to the hospital's Human Resources Department at least five (5) business days prior to the commencement date.
NOTE: This Purchase Order is valid for a period of ninety (90) days from the date of issue. Failure to commence the Nurse's deployment within this period shall render this Purchase Order null and void unless a written extension is granted by the Lagos Metropolitan Health Services Authority.For and on behalf of the Ordering Entity:
