This Purchase Order is issued by Montreal Rehabilitation & Wellness Group Inc. to formally authorize the engagement of a licensed Occupational Therapist to provide inpatient and outpatient rehabilitation services at our primary facility located in Canada Montreal. The Occupational Therapist shall be responsible for assessing, planning, and delivering evidence-based therapeutic interventions to patients recovering from neurological injuries, orthopedic surgeries, and age-related functional decline. All services rendered under this Purchase Order must comply with the standards set forth by the Ordre des ergothérapeutes du Québec (OEThQ) and the broader regulatory framework governing allied health professionals in Canada Montreal.

The Occupational Therapist engaged under this Purchase Order shall perform the following duties at the Canada Montreal facility and its affiliated outpatient clinic:

Conduct comprehensive functional assessments for all referred patients within 48 hours of admission or initial appointment.
Develop individualized treatment plans targeting activities of daily living (ADLs), instrumental ADLs, cognitive rehabilitation, and sensory integration.
Deliver direct therapeutic interventions including upper-extremity rehabilitation, hand therapy, cognitive-behavioral strategies, and environmental modification consultations.
Collaborate with multidisciplinary teams including physiotherapists, speech-language pathologists, physicians, and social workers within the Canada Montreal healthcare network.
Maintain accurate and timely clinical documentation in the facility's electronic health record system, ensuring compliance with Quebec's Loi sur les professions and privacy legislation (Law 25).
Participate in weekly case conferences and monthly quality-improvement committees specific to the Occupational Therapy department.
Ensure all therapeutic equipment and adaptive devices procured under Item #4 are properly calibrated, maintained, and documented per manufacturer specifications.

Payment Terms: Net 30 days from receipt of a valid invoice. Payment shall be made via electronic funds transfer (EFT) to the account designated by the vendor. All amounts are denominated in Canadian Dollars (CAD).
Commencement Date: The Occupational Therapist shall report for duty no later than August 4, 2025. A two-week orientation period is included in the contracted hours.
Contract Duration: This Purchase Order covers a 40-week service period from August 4, 2025, through May 23, 2026, subject to renewal by mutual written agreement.
Licensing Requirement: The Occupational Therapist must hold an active, unrestricted license in good standing with the Ordre des ergothérapeutes du Québec (OEThQ) for the entire duration of this Purchase Order. Proof of licensure and professional liability insurance (minimum $5,000,000) must be submitted prior to commencement.
Compliance: All services must adhere to the standards of practice established by the OEThQ, the Canadian Association of Occupational Therapists (CAOT), and applicable federal and provincial regulations in Canada Montreal.
Confidentiality: The Occupational Therapist is bound by strict confidentiality obligations under Quebec's Health Insurance Act and the Personal Information Protection and Electronic Documents Act (PIPEDA). A signed Non-Disclosure Agreement (NDA) is a prerequisite to service delivery.

Governing Law: This Purchase Order shall be governed by and construed in accordance with the Civil Code of Quebec and the federal laws of Canada applicable in the province of Quebec, Canada Montreal jurisdiction.
Dispute Resolution: Any disputes arising from this Purchase Order shall first be subject to good-faith mediation. If unresolved, matters shall be referred to the courts of competent jurisdiction in Canada Montreal, Quebec.

NOTE: This Purchase Order is contingent upon the successful completion of a background check (criminal record and vulnerable-sector check) for the Occupational Therapist, as required by the Canada Montreal municipal bylaws governing healthcare facilities serving minors and vulnerable adults. The vendor is responsible for coordinating and covering the cost of this check.
