1. Purchase Order Description and Purpose
This Purchase Order is issued by the Addis Ababa Regional Health Bureau, located in Ethiopia Addis Ababa, for the procurement of qualified Occupational Therapist professional services. The purpose of this Purchase Order is to engage a certified Occupational Therapist to deliver comprehensive rehabilitation, assessment, and therapeutic intervention services to patients at the Addis Ababa Public Health Institute and affiliated community health centers across the Ethiopia Addis Ababa metropolitan area. This Purchase Order is issued in accordance with the Federal Democratic Republic of Ethiopia Public Procurement and Property Administration Proclamation No. 1160/2019 and all applicable regulations governing the procurement of professional health services in Ethiopia Addis Ababa.
2. Scope of Services – Occupational Therapist
The Occupational Therapist engaged under this Purchase Order shall provide the following professional services within the Ethiopia Addis Ababa service area:

3. Terms and Conditions of This Purchase Order
- Validity: This Purchase Order is valid for a period of twelve (12) months from the date of issue, commencing 1 July 2025 and concluding 30 June 2026, unless terminated earlier in accordance with the terms herein.
- Qualification Requirements: The Occupational Therapist engaged under this Purchase Order must hold a minimum of a Bachelor's degree in Occupational Therapy from a recognized institution in Ethiopia or an internationally accredited program, and must be registered with the Ethiopian Health Professions Council. A minimum of three (3) years of post-qualification clinical experience in Ethiopia Addis Ababa or equivalent settings is required.
- Payment Terms: Payment for services rendered under this Purchase Order shall be made on a monthly basis within thirty (30) days of receipt of a valid invoice and verified service report. All payments shall be processed in Ethiopian Birr (ETB) through the Addis Ababa Regional Health Bureau's designated bank account at Commercial Bank of Ethiopia, Addis Ababa branch.
- Performance Standards: The Occupational Therapist shall adhere to the clinical practice guidelines established by the Federal Ministry of Health of Ethiopia and the specific service-level agreements outlined in the annex to this Purchase Order. Monthly performance reports must be submitted to the Procurement Director by the fifth (5th) business day of each month.
- Confidentiality: The Occupational Therapist shall maintain strict confidentiality of all patient records and clinical data in accordance with Ethiopian health data protection regulations. All patient information collected during the delivery of services in Ethiopia Addis Ababa shall be stored securely and shall not be disclosed to any third party without written consent.
- Termination: Either party may terminate this Purchase Order with a written notice of thirty (30) days. The Addis Ababa Regional Health Bureau reserves the right to terminate this Purchase Order immediately in the event of material breach, professional misconduct, or failure to meet the performance standards of the Occupational Therapist role.
- Governing Law: This Purchase Order shall be governed by and construed in accordance with the laws of the Federal Democratic Republic of Ethiopia. Any disputes arising from this Purchase Order shall be resolved through the competent courts of Ethiopia Addis Ababa.
- Force Majeure: Neither party shall be liable for failure to perform obligations under this Purchase Order due to circumstances beyond reasonable control, including but not limited to natural disasters, civil unrest, or government-mandated closures affecting operations in Ethiopia Addis Ababa.
- Insurance and Liability: The Occupational Therapist shall maintain professional indemnity insurance coverage of no less than ETB 5,000,000 for the duration of this Purchase Order. The supplier organization shall bear full liability for any professional negligence or malpractice occurring during the delivery of services.
- Amendments: Any modification or amendment to this Purchase Order must be made in writing and signed by both parties. Verbal agreements shall not constitute a valid amendment to the terms of this Purchase Order.
4. Delivery and Commencement
The Occupational Therapist services under this Purchase Order shall commence on 1 July 2025 at the Addis Ababa Public Health Institute, Bole Sub-City, Ethiopia Addis Ababa. The Occupational Therapist shall report to the Director of Rehabilitation Services and shall be available for clinical duties from Monday to Friday, 08:00 to 17:00 East Africa Time (EAT), with additional availability for community outreach programs as scheduled by the Ethiopia Addis Ababa health authority.
5. Acceptance and Authorization
By signing below, both parties acknowledge and accept all terms, conditions, and obligations set forth in this Purchase Order for the engagement of an Occupational Therapist in Ethiopia Addis Ababa. This Purchase Order constitutes a binding agreement between the parties upon execution.
