This Purchase Order is issued by Mumbai Wellness & Rehabilitation Centre Pvt. Ltd. for the engagement of a qualified Occupational Therapist to provide specialized therapeutic services to patients and clients at our facility located in India Mumbai. The Occupational Therapist shall be responsible for the assessment, planning, and delivery of individualized and group-based occupational therapy interventions for patients with neurological conditions, orthopedic injuries, developmental disabilities, and age-related functional decline.

The scope of this Purchase Order encompasses the full professional engagement of the Occupational Therapist for a period of twelve (12) months, commencing on 1st August 2025 and concluding on 31st July 2026, subject to renewal as per the terms outlined herein.

The Occupational Therapist engaged under this Purchase Order must satisfy the following mandatory qualifications and compliance requirements as applicable in India Mumbai:

Post-graduate degree in Occupational Therapy (M.O.T.) from a university recognized by the Government of India and the Rehabilitation Council of India (RCI).
Valid registration with the Maharashtra Council of Occupational Therapy and the Rehabilitation Council of India (RCI Registration No. to be provided prior to commencement).
Minimum five (5) years of post-qualification clinical experience in a hospital or rehabilitation setting in India Mumbai or equivalent metropolitan area.
Valid medical practitioner license and professional indemnity insurance with a minimum coverage of INR 50,00,000 (Fifty Lakh Rupees).
Completion of a current Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) certification.
Compliance with all data protection and patient confidentiality regulations under the Digital Personal Data Protection Act, 2023, applicable in India Mumbai.

All payments under this Purchase Order shall be made in Indian Rupees (INR) via electronic bank transfer (NEFT/RTGS) to the account designated by the Occupational Therapist. Invoices shall be submitted on the last working day of each calendar month. Payment shall be processed within fifteen (15) calendar days of receipt of a valid invoice. Applicable Goods and Services Tax (GST) at the prevailing rate of 18% shall be charged separately and is inclusive of the rates stated above. No advance payment shall be made. The Purchasing Entity reserves the right to withhold payment for any services not rendered or found to be substandard upon review.

Governing Law: This Purchase Order shall be governed by and construed in accordance with the laws of India, with jurisdiction vested in the competent courts of Mumbai, Maharashtra, India Mumbai.
Commencement: The Occupational Therapist shall report for duty on 1st August 2025 at the Mumbai Wellness & Rehabilitation Centre facility, Bandra East, India Mumbai, between the hours of 09:00 AM and 05:00 PM, Monday through Saturday.
Working Hours: The Occupational Therapist shall work a minimum of 36 hours per week. Overtime, if required, shall be compensated at 1.5 times the hourly equivalent of the monthly rate, subject to prior written approval by the Chief Operations Officer.
Leave and Holidays: The Occupational Therapist shall be entitled to 12 days of paid annual leave and all public holidays as declared by the Government of Maharashtra for the city of Mumbai.
Confidentiality: The Occupational Therapist shall maintain strict confidentiality of all patient records, clinical data, and proprietary information of the Purchasing Entity. This obligation shall survive the termination of this Purchase Order for a period of five (5) years.
Termination: Either party may terminate this Purchase Order by providing thirty (30) days' written notice. The Purchasing Entity may terminate immediately in the event of gross professional misconduct, loss of RCI registration, or material breach of this Purchase Order.
Dispute Resolution: Any dispute arising out of or in connection with this Purchase Order shall first be attempted to be resolved through amicable negotiation. Failing resolution within thirty (30) days, the dispute shall be referred to arbitration in Mumbai, India Mumbai, in accordance with the Arbitration and Conciliation Act, 1996.
Force Majeure: Neither party shall be liable for failure to perform obligations under this Purchase Order due to events beyond reasonable control, including but not limited to natural disasters, government-mandated lockdowns, or public health emergencies affecting India Mumbai.
Intellectual Property: All treatment protocols, assessment tools, and documentation developed specifically for the Purchasing Entity during the term of this Purchase Order shall remain the property of Mumbai Wellness & Rehabilitation Centre Pvt. Ltd.
Compliance with Local Regulations: The Occupational Therapist shall comply with all applicable municipal, state, and national regulations governing the practice of occupational therapy in India Mumbai, including those issued by the Municipal Corporation of Greater Mumbai (MCGM) and the Maharashtra State Health Department.

This Purchase Order constitutes a formal and binding request for the engagement of the Occupational Therapist as described herein. By signing below, both parties acknowledge and agree to all terms, conditions, and obligations set forth in this Purchase Order. This document is valid for acceptance within thirty (30) days of the date of issue. Failure to accept within the stipulated period shall render this Purchase Order null and void.
