This Purchase Order is issued by Kuala Lumpur Integrated Rehabilitation Centre Sdn. Bhd. to engage a qualified and registered Occupational Therapist to deliver comprehensive occupational therapy services within the Malaysia Kuala Lumpur region. The Occupational Therapist shall be responsible for the assessment, diagnosis, and treatment of patients with physical, cognitive, developmental, and sensory impairments. All services rendered under this Purchase Order must comply with the standards set by the Malaysian Allied Health Professions Council (MAHPC) and the Ministry of Health Malaysia (Kementerian Kesihatan Malaysia).
The Occupational Therapist is expected to maintain a valid practising certificate, carry professional indemnity insurance of no less than RM 1,000,000, and adhere to the Code of Ethics for Occupational Therapists in Malaysia. All clinical records and patient data must be stored in accordance with the Personal Data Protection Act 2010 (PDPA) of Malaysia.
- This Purchase Order is valid for acceptance within seven (7) calendar days from the date of issue. Failure to accept within this period shall render this Purchase Order null and void.
- The Occupational Therapist shall commence all services no later than 01 July 2025 at the designated facility in Malaysia Kuala Lumpur. Any delay must be communicated in writing at least fourteen (14) days prior to the start date.
- Payment shall be processed within thirty (30) days from the receipt of a valid tax invoice. Late payments shall attract interest at the rate of 5% per annum as stipulated under the Late Payment of Outstanding Payments to SMEs Act 2010 of Malaysia.
- The Occupational Therapist shall submit monthly service reports detailing patient progress, session attendance, and any clinical concerns to the Procurement Department of the purchasing entity.
- Either party may terminate this Purchase Order with thirty (30) days written notice. In the event of termination, all services rendered up to the termination date shall be invoiced and settled in full.
- All disputes arising from this Purchase Order shall be resolved through mediation in Malaysia Kuala Lumpur in accordance with the rules of the Asian International Arbitration Centre (AIAC). The governing law shall be the laws of Malaysia.
- The Occupational Therapist shall not sublet or delegate any portion of the services outlined in this Purchase Order without prior written consent from the purchasing entity.
- This Purchase Order does not constitute an employment relationship. The Occupational Therapist is engaged as an independent contractor and shall be responsible for all statutory contributions, including EPF (KWSP) and SOCSO, as applicable under Malaysian law.
