1. Purpose of This Purchase OrderThis Purchase Order is issued by the Federal Capital Territory Health Authority to formally engage the services of a qualified Occupational Therapist to provide specialized rehabilitation and therapeutic interventions at the FCT General Hospital, Nigeria Abuja. This Purchase Order serves as the binding contractual instrument governing the scope, duration, compensation, and professional obligations of the Occupational Therapist engagement. All terms and conditions outlined in this Purchase Order are subject to the Federal Government of Nigeria procurement regulations and the National Health Act of 2014.
2. Scope of Services – Occupational TherapistThe Occupational Therapist engaged under this Purchase Order shall provide the following professional services at the designated facility in Nigeria Abuja:
- Assessment and evaluation of patients with physical, neurological, developmental, and psychiatric conditions requiring occupational therapy intervention.
- Development and implementation of individualized treatment plans for patients recovering from stroke, traumatic brain injury, spinal cord injuries, and orthopedic conditions.
- Provision of adaptive equipment training, fine motor skill development, and activities of daily living (ADL) retraining for inpatients and outpatients.
- Supervision and mentorship of junior allied health staff and student Occupational Therapist trainees from the University of Nigeria, Nsukka, and Ahmadu Bello University.
- Participation in multidisciplinary team meetings and case conferences at the FCT General Hospital, Nigeria Abuja, no fewer than three (3) times per week.
- Preparation of clinical documentation, progress reports, and discharge summaries in accordance with Nigerian Health Facility standards.
- Conduct of community outreach programs in underserved areas of Nigeria Abuja, including Gwagwalada, Kuje, and Bwari local government areas, on a bi-weekly basis.

- Payment shall be made in monthly installments within fourteen (14) working days of the Occupational Therapist submitting a duly signed timesheet and service delivery report to the FCT Health Authority, Nigeria Abuja.
- All payments shall be processed via bank transfer to the account designated by Therapeutic Excellence Nigeria Ltd. as stated in this Purchase Order.
- A retention of five percent (5%) shall be withheld from each monthly payment and released upon satisfactory completion of the full twelve-month contract period.
- The Occupational Therapist shall be required to submit a valid Tax Clearance Certificate (TCC) from the Federal Inland Revenue Service (FIRS), Nigeria Abuja office, before the first payment is processed.
- Any variation in scope of services must be approved in writing via a formal amendment to this Purchase Order before additional charges are incurred.
This Purchase Order shall be effective from 1 July 2025 through 30 June 2026, constituting a twelve (12) month engagement period for the Occupational Therapist services in Nigeria Abuja. Either party may terminate this Purchase Order with a written notice of thirty (30) days. In the event of termination, the Occupational Therapist shall be compensated for all services rendered up to the effective date of termination, less applicable statutory deductions. The FCT Health Authority reserves the right to terminate this Purchase Order immediately in the event of gross professional misconduct, loss of AHPCN registration, or failure to meet the performance benchmarks outlined in the service level agreement annexed to this document.
6. Professional Requirements and Compliance- The Occupational Therapist must hold a valid Practicing License issued by the Allied Health Professions Council of Nigeria (AHPCN) and a minimum of a Bachelor of Science in Occupational Therapy (B.Sc. OT) or equivalent qualification.
- The Occupational Therapist must maintain a minimum of 20 units of Continuing Professional Development (CPD) credits annually as mandated by the AHPCN.
- All clinical records and patient data shall be handled in strict compliance with the Nigeria Data Protection Regulation (NDPR) 2019 and the National Health Act 2014.
- The Occupational Therapist shall comply with all institutional policies, infection control protocols, and safety regulations of the FCT General Hospital, Nigeria Abuja.
- A valid National Identity Number (NIN) and proof of residence in Nigeria Abuja or the Federal Capital Territory shall be submitted prior to commencement of duties.
Any dispute arising from the interpretation or execution of this Purchase Order shall first be subject to internal mediation by the FCT Health Authority, Nigeria Abuja. Should mediation fail to resolve the matter within thirty (30) days, the dispute shall be referred to arbitration in accordance with the Arbitration and Conciliation Act, Cap A18, Laws of the Federation of Nigeria 2004. The seat of arbitration shall be Nigeria Abuja, and the language of proceedings shall be English.
8. Authorization and SignaturesThis Purchase Order is authorized and executed by the undersigned representatives of both parties, acknowledging full understanding and acceptance of all terms, conditions, and obligations herein.
