This Purchase Order is issued by Islamabad Rehabilitation & Allied Health Centre for the engagement of a qualified Occupational Therapist to provide specialized therapeutic services to patients within the Islamabad, Pakistan facility. The Occupational Therapist shall deliver evidence-based interventions targeting motor function restoration, sensory integration, cognitive rehabilitation, and adaptive equipment training for patients aged 3 to 85 years. All services shall be rendered in compliance with the Allied Health Professions Act, 2011, and the standards set by the Allied Health Practitioners Council of Pakistan (AHPCP) operating in Islamabad, Pakistan.

- This Purchase Order is governed by the laws of the Islamic Republic of Pakistan and shall be interpreted in accordance with the Federal Capital Territory regulations applicable in Islamabad, Pakistan.
- The Occupational Therapist must hold a valid registration with the Allied Health Practitioners Council of Pakistan (AHPCP) and a recognized Bachelor's or Master's degree in Occupational Therapy from a university approved by the Higher Education Commission (HEC) of Pakistan.
- All Occupational Therapist services shall be delivered at the Islamabad Rehabilitation & Allied Health Centre premises located in Blue Area, Islamabad, Pakistan, unless otherwise authorized in writing by the Purchasing Party.
- The Occupational Therapist shall maintain complete and confidential patient records in accordance with the Pakistan Data Protection Act, 2023, and shall not disclose any patient information to third parties without written consent.
- Payment shall be made on a monthly basis within 30 days of receipt of a valid invoice from the Occupational Therapist. Payment shall be processed via bank transfer to the account specified by the Occupational Therapist in Islamabad, Pakistan.
- The Occupational Therapist is responsible for procuring all therapeutic materials, adaptive equipment, and assistive devices required for treatment sessions, unless specifically stated otherwise in this Purchase Order.
- Either party may terminate this Purchase Order with 30 days' written notice. In the event of termination, the Purchasing Party shall compensate the Occupational Therapist for services rendered up to the date of termination.
- The Occupational Therapist shall carry professional indemnity insurance with a minimum coverage of PKR 5,000,000, valid throughout the duration of this Purchase Order in Islamabad, Pakistan.
- Any disputes arising from this Purchase Order shall be resolved through arbitration in Islamabad, Pakistan, in accordance with the Arbitration Act, 1940 of Pakistan.
- This Purchase Order constitutes the entire agreement between the parties regarding the engagement of the Occupational Therapist and supersedes all prior negotiations, representations, or agreements.
The Occupational Therapist shall commence services no later than 01 July 2025 at the Islamabad facility. The total contract period under this Purchase Order shall be twelve (12) months, extendable by mutual written agreement of both parties. The Occupational Therapist shall report to the Chief Medical Officer of the Islamabad Rehabilitation & Allied Health Centre on the first working day of each month for performance review.
7. Acceptance and AuthorizationBy signing below, both parties acknowledge and accept all terms, conditions, and obligations outlined in this Purchase Order for the engagement of the Occupational Therapist in Islamabad, Pakistan. This document is valid only upon the authorized signatures of both the Purchasing Party and the Occupational Therapist service provider.
