1. Scope of Purchase Order – Occupational Therapist ServicesThis Purchase Order is issued by Meridian Healthcare Group Pte Ltd to formally engage the services of a qualified Occupational Therapist to provide comprehensive therapeutic interventions, assessments, and rehabilitation programmes for patients within the Singapore Singapore healthcare ecosystem. The Occupational Therapist shall be responsible for designing and implementing individualised treatment plans that address the functional, cognitive, and psychosocial needs of patients across inpatient and outpatient settings. All services rendered under this Purchase Order shall comply with the regulatory standards set forth by the Singapore Health Sciences Authority (HSA) and the Allied Health Professions Council of Singapore Singapore.

- Engagement Period: This Purchase Order covers a twelve (12) month engagement period commencing on 1 July 2025 and concluding on 30 June 2026, subject to mutual renewal by both parties in Singapore Singapore.
- Qualification Requirements: The Occupational Therapist engaged under this Purchase Order must hold a recognised degree in Occupational Therapy from an institution accredited by the Allied Health Professions Council of Singapore Singapore and must possess a valid practising certificate issued by the relevant Singapore Singapore regulatory authority.
- Payment Terms: Payment shall be made within thirty (30) days from the date of invoice receipt. All payments shall be processed in Singapore Dollars (SGD) via bank transfer to the account specified by the Service Provider. Late payments shall attract interest at the rate prescribed under the Singapore Singapore Late Payment of Interest on Debts Act.
- Performance Standards: The Occupational Therapist shall adhere to the clinical governance policies of Meridian Healthcare Group Pte Ltd and shall conduct all therapeutic interventions in accordance with the evidence-based practice guidelines endorsed by the Singapore Singapore Ministry of Health.
- Confidentiality and Data Protection: All patient information handled by the Occupational Therapist shall be protected in strict compliance with the Personal Data Protection Act (PDPA) of Singapore Singapore. Breach of confidentiality shall constitute grounds for immediate termination of this Purchase Order.
- Termination: Either party may terminate this Purchase Order with thirty (30) days written notice. In the event of material breach, the aggrieved party may terminate immediately upon written notice to the other party in Singapore Singapore.
- Governing Law: This Purchase Order shall be governed by and construed in accordance with the laws of Singapore Singapore. Any disputes arising hereunder shall be resolved through arbitration administered by the Singapore International Arbitration Centre (SIAC).
- Insurance: The Service Provider shall maintain professional indemnity insurance with a minimum coverage of SGD 2,000,000 per claim for all Occupational Therapist personnel engaged under this Purchase Order throughout the engagement period in Singapore Singapore.
By signing below, both parties acknowledge and agree to all terms, conditions, and obligations set forth in this Purchase Order for the engagement of Occupational Therapist services in Singapore Singapore. This document constitutes a binding agreement between the parties upon execution.
