This Purchase Order is issued by Wellness Rehabilitation & Therapy Services (Pvt) Ltd. to formally engage the services of a qualified Occupational Therapist to provide comprehensive therapeutic interventions, patient assessments, and rehabilitation programmes at our primary facility located in Sri Lanka Colombo. The Occupational Therapist shall deliver all services in accordance with the standards set forth by the Sri Lanka Occupational Therapy Association and the Ministry of Health, Sri Lanka. All clinical sessions, patient evaluations, and therapeutic planning shall be conducted at the designated facility in Sri Lanka Colombo unless otherwise agreed in writing by both parties.

1. This Purchase Order constitutes a binding agreement between Wellness Rehabilitation & Therapy Services (Pvt) Ltd. and the named Occupational Therapist upon written acceptance by both parties. The Occupational Therapist shall confirm acceptance of this Purchase Order within five (5) business days of receipt.
2. All services described in this Purchase Order shall be rendered at the facility located at 42, Galle Road, Colombo 03, Sri Lanka Colombo, or at such other location within Sri Lanka Colombo as mutually agreed in writing. Home visits shall be limited to the Colombo Municipal Council and Colombo District boundaries.
3. The Occupational Therapist shall maintain valid professional registration with the Sri Lanka Occupational Therapy Association and shall provide proof of registration and professional indemnity insurance (minimum coverage of LKR 5,000,000) prior to commencing any services under this Purchase Order.
4. Payment shall be made within thirty (30) calendar days from the date of receipt of a valid tax invoice. Invoices shall reference this Purchase Order number (PO/2025/OT/0047) and shall be submitted to the accounts department at Wellness Rehabilitation & Therapy Services (Pvt) Ltd., Sri Lanka Colombo.
5. The Occupational Therapist shall maintain strict patient confidentiality in accordance with the Sri Lanka Data Protection Act No. 9 of 2022. All patient records, assessment reports, and treatment documentation generated under this Purchase Order shall remain the property of Wellness Rehabilitation & Therapy Services (Pvt) Ltd.
6. Any modification, amendment, or cancellation of this Purchase Order must be made in writing and signed by authorised representatives of both parties. Verbal agreements shall not be binding.
7. The Occupational Therapist shall adhere to all clinical protocols, safety guidelines, and regulatory requirements applicable to therapeutic services in Sri Lanka Colombo as prescribed by the Ministry of Health and the relevant professional bodies.
8. In the event of non-performance or breach of any term of this Purchase Order, the purchasing party reserves the right to terminate the engagement with fourteen (14) days written notice and to seek remedies as provided under the Contract Act of Sri Lanka.
9. This Purchase Order shall be governed by and construed in accordance with the laws of the Democratic Socialist Republic of Sri Lanka. Any disputes arising shall be subject to the exclusive jurisdiction of the courts in Colombo, Sri Lanka Colombo.

The Occupational Therapist shall commence services on or before 1 July 2025 and shall continue to provide services as outlined in this Purchase Order for a period of six (6) months, concluding on 31 December 2025, unless extended by mutual written agreement. The Occupational Therapist is expected to be available at the Sri Lanka Colombo facility a minimum of five (5) days per week, Monday through Friday, between the hours of 08:00 and 17:00, with the option of Saturday morning sessions by prior arrangement.
