This Purchase Order authorizes the procurement of specialized Occupational Therapist services to be rendered at the Gulf Coast Rehabilitation & Wellness Center facility located in United States Houston. The contracted Occupational Therapist shall provide comprehensive, evidence-based therapeutic interventions to patients recovering from orthopedic injuries, neurological conditions, and post-surgical rehabilitation. All services must comply with the standards set forth by the American Occupational Therapy Association (AOTA) and the Texas Department of State Health Services.

The Occupational Therapist is expected to conduct individualized assessments, develop personalized treatment plans, administer therapeutic exercises, recommend adaptive equipment, and provide caregiver education. The scope includes a minimum of 32 clinical hours per week, with availability for on-call consultation during extended business hours at the United States Houston facility.

  • Payment shall be made within thirty (30) days of receipt of a valid invoice from the Occupational Therapist service provider.
  • Invoices must reference this Purchase Order number (HOU-OT-2025-04872) and include itemized hours or services rendered during the billing period.
  • Payment method: Electronic Funds Transfer (EFT) to the account designated by Bayou City Therapeutic Partners, Inc.
  • A late payment penalty of 1.5% per month shall apply to any undisputed balance exceeding the net-30 window.
  • All amounts are in United States Dollars and are subject to applicable Texas state and federal tax regulations.
  • This Purchase Order is governed by the laws of the State of Texas and the federal laws of the United States. Any disputes shall be resolved through binding arbitration in United States Houston, Harris County, Texas.
  • The Occupational Therapist must maintain current professional licensure, malpractice insurance (minimum $2,000,000 per occurrence), and HIPAA compliance throughout the contract term.
  • The Buyer reserves the right to audit clinical records, treatment documentation, and billing accuracy with forty-eight (48) hours written notice.
  • Either party may terminate this Purchase Order with sixty (60) days written notice. In the event of material breach, termination may be effective immediately upon written notice.
  • All patient data collected during Occupational Therapist sessions in United States Houston shall be stored in compliance with the Health Insurance Portability and Accountability Act (HIPAA) and Texas Medical Records Privacy Act.
  • This Purchase Order does not constitute an employment relationship. The Occupational Therapist is engaged as an independent contractor or through a corporate service agreement.
  • Force majeure events, including but not limited to natural disasters affecting the United States Houston metropolitan area, shall excuse timely performance for the duration of the event plus a reasonable recovery period.

By signing below, both parties acknowledge and agree to all terms, conditions, and specifications outlined in this Purchase Order for Occupational Therapist services to be performed in United States Houston. This document constitutes a binding agreement effective as of the date of the last signature.
