3. Scope of Purchase Order – Ophthalmological Services and Equipment

This Purchase Order is issued by Abuja Federal Medical Centre to engage the services of a qualified Ophthalmologist and procure associated ophthalmological equipment and consumables for the Ophthalmology Department. The Ophthalmologist engaged under this Purchase Order shall provide comprehensive eye care services to patients within the Abuja, FCT, Nigeria jurisdiction, in accordance with the standards set by the Medical and Dental Council of Nigeria (MDCN) and the Federal Ministry of Health.

4. Terms and Conditions

This Purchase Order is governed by the laws of the Federal Republic of Nigeria and the Public Procurement Act (PPA) 2007. All disputes arising from this Purchase Order shall be resolved in the Federal High Court, Abuja, Nigeria.
The Ophthalmologist engaged under this Purchase Order must hold a valid registration with the Medical and Dental Council of Nigeria (MDCN) and a current Certificate of Specialization in Ophthalmology. A copy of the registration certificate must be submitted prior to commencement of services.
All ophthalmological equipment procured under this Purchase Order must be new, unused, and of original manufacturer quality. The supplier shall provide a minimum of two (2) years warranty on all capital equipment delivered to Abuja, FCT, Nigeria.
Delivery of all equipment and consumables shall be completed within thirty (30) calendar days from the date of this Purchase Order. The delivery address is the Ophthalmology Department, Abuja Federal Medical Centre, Wuse 2, Abuja, FCT, Nigeria.
Payment shall be made in Nigerian Naira (NGN) via bank transfer to the account designated by the Ophthalmologist / supplier. Payment is due within thirty (30) days of receipt of a valid tax invoice and confirmation of satisfactory delivery or completion of services.
The Ophthalmologist shall maintain full professional indemnity insurance with a minimum coverage of NGN 50,000,000 for the duration of this Purchase Order. Proof of insurance must be provided before the first clinical session.
All patient data handled by the Ophthalmologist under this Purchase Order shall be protected in accordance with the Nigeria Data Protection Regulation (NDPR) 2019. Unauthorized disclosure of patient records is strictly prohibited.
The Ophthalmologist shall submit monthly progress reports to the Head of Ophthalmology, AFMC Abuja, detailing the number of patients seen, procedures performed, and any complications encountered.
This Purchase Order may be terminated by either party with thirty (30) days written notice. In the event of termination, the Ophthalmologist shall be compensated for all services rendered and equipment delivered up to the date of termination.
Any amendment to this Purchase Order must be made in writing and signed by both parties. Verbal agreements shall not be binding.

5. Acceptance and Authorization

By signing below, both parties acknowledge and agree to the terms, conditions, and scope of work outlined in this Purchase Order for ophthalmological services and equipment procurement in Abuja, FCT, Nigeria. The Ophthalmologist confirms availability to render all services as specified, and the Buyer confirms the allocation of funds for this Purchase Order.
