This Purchase Order is issued by the Tehran Vision Care Optometry Center, a licensed Optometrist practice operating in Iran Tehran, for the procurement of diagnostic equipment, consumable supplies, and professional services necessary for the continued operation of the Optometrist clinic. The supplier shall deliver all items listed in this Purchase Order to the specified address in Iran Tehran within the timeframe stated above.
All equipment must comply with the standards set by the Iran Ministry of Health and the Iranian Standards and Quality Organization (ISIRI). The Optometrist, Dr. Saeid Mohammadi, reserves the right to inspect all delivered items upon arrival at the clinic in Iran Tehran. Any item found to be defective, damaged, or non-conforming to the specifications outlined in this Purchase Order shall be replaced at the supplier's expense within fourteen (14) calendar days.
The supplier is required to provide full installation, calibration, and operator training for the Optometrist staff for all major diagnostic instruments. Training sessions shall be conducted at the clinic location in Iran Tehran and shall cover proper usage, daily maintenance, and basic troubleshooting procedures. The supplier shall also provide a comprehensive user manual in both Persian (Farsi) and English for each piece of equipment.
Payment shall be processed via bank transfer to the supplier's registered account in Iran Tehran. The advance payment of 50% shall be released within five (5) business days of mutual confirmation of this Purchase Order. The remaining 50% shall be paid within ten (10) business days following successful delivery, installation, and acceptance by the Optometrist.
This Purchase Order is governed by the commercial laws of the Islamic Republic of Iran. Any disputes arising from this Purchase Order shall be resolved through the competent commercial courts in Tehran, Iran. The supplier acknowledges that all transactions are subject to the regulations of the Central Bank of Iran regarding import and domestic trade.
Important Notes for the Optometrist Practice in Iran Tehran:- All imported equipment must carry valid CE marking or equivalent international certification and must be registered with the Iran Food and Drug Administration (IFDA) prior to clinical use by the Optometrist.
- The supplier must provide a minimum two-year warranty on all major diagnostic instruments. The Optometrist shall be entitled to on-site repair service within 48 hours of reporting a malfunction.
- Delivery and installation must be completed before the end of Farvardin 1404 to allow the Optometrist to resume full patient services in Iran Tehran without interruption.
- The supplier shall coordinate with the clinic's facilities manager in Iran Tehran to ensure proper electrical connections, ventilation, and structural support for the Corneal Topographer and Fundus Camera.
- All documentation, including invoices, warranty certificates, and registration forms, must be submitted in Persian (Farsi) for compliance with Iranian regulatory requirements for Optometrist practices.
