General Terms Governing This Purchase Order for Optometrist Services in Kuwait, Kuwait City
- This Purchase Order (PO-KW-2025-0472) constitutes a binding agreement between Al-Nour Vision Care Center, a licensed Optometrist practice operating in Kuwait, Kuwait City, and Gulf Optical Instruments LLC for the supply of optical diagnostic equipment and consumables.
- All equipment listed in this Purchase Order must comply with the standards set by the Kuwait Ministry of Health and the Kuwait Standards and Metrology Organization (KSMO). The Optometrist team at Al-Nour Vision Care Center will inspect all deliveries upon arrival at the facility in Kuwait, Kuwait City.
- Delivery shall be made to the Optometrist clinic premises located in Al-Salimiyah, Kuwait, Kuwait City, no later than 30 June 2025. The supplier is responsible for all shipping, handling, and installation costs within Kuwait, Kuwait City.
- Payment of the total amount of KWD 48,997.20 shall be made within thirty (30) calendar days from the date of confirmed delivery and acceptance by the Lead Optometrist at the facility in Kuwait, Kuwait City. Payment shall be processed via bank transfer to the supplier's designated account.
- All equipment is warranted for a minimum of twenty-four (24) months from the date of installation. The Optometrist practice reserves the right to reject any item that does not meet the specifications outlined in this Purchase Order.
- The supplier shall provide full training for the Optometrist staff and technicians at the Kuwait, Kuwait City location for all diagnostic instruments listed in this Purchase Order. Training sessions shall be conducted in English and Arabic.
- This Purchase Order is governed by the laws of the State of Kuwait. Any disputes arising from this Purchase Order shall be resolved through the competent courts in Kuwait, Kuwait City.
- Neither party may assign this Purchase Order without the prior written consent of the other party. This Purchase Order may only be amended through a written addendum signed by both the Optometrist practice representative and the supplier's authorized signatory.
- The supplier acknowledges that the equipment procured under this Purchase Order is intended exclusively for use by a licensed Optometrist and associated clinical staff at the Al-Nour Vision Care Center in Kuwait, Kuwait City, and shall not be resold or transferred without written authorization.
