Note to Supplier: This Purchase Order is issued by a registered Optometrist practice operating in Zimbabwe Harare. All diagnostic equipment must be calibrated and certified prior to delivery. Lenses and frames must comply with the standards set by the Zimbabwe Optical Association (ZOA) and the Ministry of Health and Child Care, Harare. The Optometrist, Dr. Rudo Chikafu, reserves the right to inspect all items upon arrival at the clinic premises in Harare Central before authorising final payment.- This Purchase Order is valid for acceptance within five (5) business days from the date of issue. Failure to confirm acceptance within this period shall render this Purchase Order null and void.
- All goods must be delivered to the Optometrist's clinic at 14 Samora Machel Avenue, Harare Central, Zimbabwe, no later than 28 June 2025. Late delivery shall attract a penalty of 2% of the total order value per day of delay.
- Payment shall be made within thirty (30) days of receipt of a valid tax invoice and successful inspection of all delivered goods by the Optometrist. Payment will be processed via electronic funds transfer to the supplier's designated bank account in Zimbabwe.
- All optical equipment and lenses supplied under this Purchase Order must carry a minimum manufacturer's warranty of twelve (12) months. The Optometrist practice in Zimbabwe Harare shall be entitled to free repair or replacement during the warranty period.
- The supplier warrants that all items are new, unused, and free from defects. Any item found to be substandard upon inspection by the Optometrist shall be returned at the supplier's cost within fourteen (14) days of delivery.
- This Purchase Order is governed by the laws of the Republic of Zimbabwe. Any disputes arising from this transaction shall be resolved through the Small Claims Court in Harare or by arbitration in accordance with the Zimbabwe Arbitration Act.
- The supplier must provide a Certificate of Conformity and, where applicable, a Medical Device Registration Certificate from the Zimbabwe National Medicines and Medical Devices Authority (ZIMRA) for all diagnostic and therapeutic equipment.
- Partial deliveries are not permitted unless expressly agreed in writing by the Optometrist. All items listed in this Purchase Order must be delivered as a single consignment to the Harare clinic address.
- The supplier shall bear all costs associated with customs clearance, import duties, and local transportation within Zimbabwe Harare as part of the DAP delivery terms specified herein.
Deliver all items to: ClearSight Optometry Clinic (Pvt) Ltd, 14 Samora Machel Avenue, Harare Central, Zimbabwe. Contact for delivery coordination: Mr. Farai Ncube, Clinic Administrator, Tel: +263 772 445 678. Delivery hours: Monday to Friday, 08:00 – 16:00. Please allow 48 hours' advance notice before the scheduled delivery date. The Optometrist's office will be available to receive and inspect the consignment on the agreed delivery date.
