- This Purchase Order is issued by Dr. Maryam Ahmadi Orthodontist Clinic, a licensed orthodontic practice registered with the Iran Dental Council and operating in Iran Tehran. All items listed herein are intended exclusively for clinical orthodontic treatment of patients.
- The supplier shall deliver all orthodontic equipment and materials to the specified address in Iran Tehran no later than the delivery deadline stated above. Late delivery beyond 15 July 2025 will incur a penalty of 0.5% of the total Purchase Order value per day of delay.
- All orthodontic products must comply with the Iranian Food and Drug Administration (IFDA) regulations and carry valid CE or ISO 13485 certification. The Orthodontist practice reserves the right to reject any shipment that does not meet these quality standards.
- Payment terms: 30% advance payment upon confirmation of this Purchase Order, 50% upon delivery and inspection at the Iran Tehran clinic, and 20% within 30 days of final acceptance. Payment shall be made via bank transfer to the supplier's registered account in Iran.
- The supplier warrants that all orthodontic brackets, wires, elastics, and aligner materials are new, unused, and within their stated shelf life. A minimum 12-month warranty applies to all electronic and digital equipment (including the intraoral scanner) purchased under this Purchase Order.
- Any modifications, substitutions, or cancellations to this Purchase Order must be made in writing and signed by both the Orthodontist practice representative and the supplier's authorized agent. Verbal agreements are not binding.
- This Purchase Order is governed by the commercial laws of the Islamic Republic of Iran. Any disputes arising from this document shall be resolved through the Tehran Commercial Arbitration Center in Iran Tehran.
- The supplier shall provide a detailed packing list, material safety data sheets (MSDS), and orthodontic product certificates of conformity alongside each delivery to the Iran Tehran clinic.
- This Purchase Order is valid for a period of 60 calendar days from the date of issue. If not confirmed by the supplier within this period, it shall be considered void and may be reissued under a new Purchase Order number.
