Important Note: All orthodontic materials listed in this Purchase Order must comply with the Pharmaceuticals and Medical Devices Act (PMDA) regulations of Japan Tokyo. The vendor is required to provide valid PMDA certification documents and Japanese-language labeling for all items prior to delivery. The Lead Orthodontist at Tokyo Advanced Orthodontic Center reserves the right to inspect all goods upon arrival at the Shibuya-ku facility and reject any items that do not meet the specified quality standards or regulatory requirements.

Terms and Conditions

This Purchase Order is issued by Tokyo Advanced Orthodontic Center, a licensed orthodontic practice registered in Japan Tokyo, and constitutes a binding procurement request to Nippon OrthoSupply Co., Ltd.
All items must be delivered to the specified Japan Tokyo address no later than July 15, 2025. Late delivery beyond five (5) business days will result in a penalty of 1% of the total order value per day of delay.
Payment shall be made via bank transfer to the vendor's designated account within forty-five (45) calendar days from the date of confirmed receipt and acceptance of all goods by the Lead Orthodontist.
The vendor warrants that all orthodontic products are new, unused, and manufactured in compliance with Japanese medical device standards. A minimum two-year warranty applies to all equipment items (Items 004 and 010).
Any substitutions or changes to the items listed in this Purchase Order require prior written approval from Dr. Yuki Sato, Lead Orthodontist, and must be documented via a formal Purchase Order amendment.
All intellectual property, software licenses, and technical documentation associated with the digital imaging equipment shall be transferred to Tokyo Advanced Orthodontic Center upon full payment.
This Purchase Order is governed by the laws of Japan Tokyo and the jurisdiction of the Tokyo District Court. Any disputes arising from this document shall be resolved through mediation in Japan Tokyo before litigation.
The vendor shall maintain full insurance coverage for all goods in transit within the Japan Tokyo metropolitan area and shall be liable for any damage or loss until the items are signed for by the receiving Orthodontist or designated staff member.
Confidentiality: All patient data, treatment protocols, and proprietary orthodontic methodologies referenced in connection with this Purchase Order are strictly confidential and shall not be disclosed to any third party.
This Purchase Order is valid for acceptance within ten (10) business days from the date of issue. Failure to confirm acceptance within this period will render this document void.
