This Purchase Order is issued by Dr. Ayesha Khan Orthodontic Clinic, a registered Orthodontist practice operating in Pakistan Islamabad, and is governed by the laws of the Islamic Republic of Pakistan. All transactions under this Purchase Order shall comply with the regulations set forth by the Pakistan Medical & Dental Council (PMDC) and the Federal Board of Revenue (FBR) for medical device procurement in Islamabad.
- Delivery: All items listed in this Purchase Order must be delivered to the Orthodontist clinic address in F-7/2, Islamabad, Pakistan, no later than 30 June 2025. Late delivery beyond 5 business days entitles the Orthodontist practice to a 2% penalty per week on the outstanding value.
- Quality Assurance: All orthodontic materials and equipment must carry valid CE marking or FDA clearance documentation. The Orthodontist practice reserves the right to reject any item that does not meet the specified standards or lacks proper import documentation for Pakistan Islamabad customs clearance.
- Warranty: The digital intraoral scanner and intraoral cameras carry a minimum 2-year manufacturer warranty. All orthodontic brackets and wires carry a 6-month defect replacement warranty. Warranty service must be available within the Pakistan Islamabad metropolitan area.
- Payment: Payment of PKR 2,479,400 shall be made via bank transfer to the supplier's designated account within 30 days of confirmed delivery and acceptance of all items by the Orthodontist practice. A valid Sales Tax Invoice (STI) compliant with FBR Pakistan Islamabad requirements must accompany the delivery.
- Regulatory Compliance: The supplier warrants that all orthodontic products are registered with the Drug Regulatory Authority of Pakistan (DRAP) and are legally importable and distributable within Pakistan Islamabad. Any regulatory non-compliance shall render this Purchase Order void at the sole discretion of the Orthodontist practice.
- Installation and Training: The supplier shall provide on-site installation of the digital intraoral scanner and a minimum 4-hour training session for the Orthodontist and two assistant staff members at the clinic in Islamabad. Training materials in English and Urdu shall be provided.
- Dispute Resolution: Any disputes arising from this Purchase Order shall be resolved through arbitration in Islamabad, Pakistan, in accordance with the Arbitration Act 1940 of Pakistan. The jurisdiction of the courts of Islamabad shall be exclusive.
- Confidentiality: Both parties agree to maintain confidentiality regarding patient data, pricing structures, and business practices related to this Orthodontist practice in Pakistan Islamabad.
