This Purchase Order is issued by PT. The Pharmacist shall be responsible for the dispensing of prescription medications, providing patient counseling on drug interactions and dosage regimens, managing inventory of pharmaceutical products, ensuring compliance with the Indonesian Ministry of Health (Kementerian Kesehatan RI) regulations, and overseeing the quality control of all medicines and health products handled at the facility.

The Pharmacist engaged under this Purchase Order must hold a valid Apoteker license issued by the Indonesian Pharmacy Council (Dewan Apoteker Indonesia) and possess a current Surat Izin Praktik Apoteker (SIPA) registered with the local health authority in Indonesia Jakarta. The Pharmacist shall also be required to complete a minimum of forty (40) hours of Continuing Professional Development (CPD) annually as mandated by national pharmaceutical regulations.

4.1 This Purchase Order is governed by the laws of the Republic of Indonesia, specifically the Indonesian Civil Code (KUHPerdata) and applicable pharmaceutical regulations issued by the Ministry of Health of Indonesia Jakarta.

4.2 The Pharmacist shall commence duties no later than 1 August 2025 at the designated Indonesia Jakarta facility. Failure to commence within the stipulated period shall entitle the Buyer to cancel this Purchase Order without penalty.

4.3 Payment shall be made in monthly installments via bank transfer to the Supplier's designated account within fifteen (15) calendar days from the date of invoice submission. The first payment shall be due upon successful onboarding and verification of the Pharmacist's credentials.

4.4 The Supplier guarantees that the Pharmacist engaged under this Purchase Order is free from any disciplinary actions, has no history of malpractice, and maintains full professional indemnity insurance coverage throughout the engagement period.

4.5 Any modification to the scope of services, working hours, or deployment location within Indonesia Jakarta must be documented in a written amendment to this Purchase Order, signed by both parties.

4.6 The Pharmacist shall adhere to all standard operating procedures (SOPs) established by PT. Nusantara Medika Sejahtera and comply with the Good Pharmacy Practice (GPP) guidelines as enforced in Indonesia Jakarta.

4.7 In the event of termination, either party may provide thirty (30) days written notice. Upon termination, all company property, software licenses, and confidential patient data in the possession of the Pharmacist must be returned within five (5) business days.

The Pharmacist shall report for duty at the following location: PT. Nusantara Medika Sejahtera, Retail Pharmacy Outlet No. 7, Jl. HR. Rasuna Said Kav. 1-2, Kuningan, Indonesia Jakarta 12950. Working hours shall be Monday through Saturday, 08:00 to 17:00 WIB (Western Indonesia Time), with a one-hour lunch break. The Pharmacist shall be available for on-call emergency dispensing support on a rotational basis as determined by the Buyer's operations manager.

By signing below, both parties acknowledge and agree to all terms, conditions, and specifications outlined in this Purchase Order for the procurement of Pharmacist services in Indonesia Jakarta.

This Purchase Order document is issued in duplicate. One copy is retained by the Buyer and one copy is provided to the Supplier. This document constitutes a legally binding agreement for the procurement of Pharmacist professional services in Indonesia Jakarta. All disputes arising from this Purchase Order shall be resolved through the Jakarta District Court (Pengadilan Negeri Jakarta Selatan) in accordance with Indonesian law.
