This Purchase Order is issued by Golden Lotus Pharmacy & Healthcare Center, a registered pharmaceutical retail and consulting establishment operating in Myanmar Yangon, for the procurement of professional Pharmacist services. The purpose of this Purchase Order is to formally engage a qualified and licensed Pharmacist to provide comprehensive pharmaceutical care, medication management, clinical consultation, and regulatory compliance oversight at our Myanmar Yangon facility. The Pharmacist engaged under this Purchase Order shall be responsible for ensuring that all dispensing activities, drug interactions assessments, patient counseling sessions, and inventory quality controls are conducted in strict accordance with the Myanmar Pharmacy Law and the regulations set forth by the Ministry of Health and Sports of the Republic of the Union of Myanmar.

- This Purchase Order constitutes a binding agreement between the Purchasing Entity and the Pharmacist service provider upon written acknowledgment and signature by both parties. The Pharmacist must hold a valid license issued by the Myanmar Pharmacy Council and must maintain that license throughout the entire term of this Purchase Order.
- The Pharmacist shall perform all duties in compliance with the Myanmar Pharmacy Law, the Drug and Food Authority regulations applicable in Myanmar Yangon, and all ethical standards prescribed by the Myanmar Pharmacy Council. Any violation of these regulatory frameworks shall constitute grounds for immediate termination of this Purchase Order.
- Payment shall be processed via bank transfer to the Pharmacist's designated account in Myanmar Kyat (MMK) within thirty (30) calendar days of the end of each monthly service period. Late payments shall incur an interest charge of 1.5% per month as stipulated under Myanmar commercial law.
- The Pharmacist is required to maintain professional indemnity insurance with a minimum coverage of 50,000,000 MMK, valid throughout the duration of this Purchase Order. Proof of insurance must be submitted to the Purchasing Entity prior to the commencement of services.
- All patient records, prescription data, and pharmaceutical information handled by the Pharmacist under this Purchase Order shall be treated as strictly confidential in accordance with Myanmar data protection guidelines. Unauthorized disclosure of patient information in the Myanmar Yangon jurisdiction shall result in immediate contract termination and legal action.
- This Purchase Order may be terminated by either party with a written notice of thirty (30) days. In the event of termination, the Pharmacist shall complete all pending dispensing obligations and hand over all inventory records to the Purchasing Entity within the notice period.
- Any disputes arising from this Purchase Order shall be resolved through mediation in Myanmar Yangon in accordance with the Myanmar Arbitration Law. The courts of Myanmar Yangon shall have exclusive jurisdiction over any matters not resolved through mediation.
- The Pharmacist shall attend all mandatory training sessions organized by the Myanmar Pharmacy Council and the Myanmar Yangon Health Department. The cost of such training, where not covered by the Purchasing Entity, shall be borne by the Pharmacist unless otherwise agreed in writing.
- This Purchase Order is governed by and shall be interpreted in accordance with the laws of the Republic of the Union of Myanmar. All references to "Myanmar Yangon" in this document refer to the administrative and operational jurisdiction of the Yangon Region.
By signing below, both parties acknowledge that they have read, understood, and agree to all terms, conditions, and obligations set forth in this Purchase Order for the engagement of Pharmacist services in Myanmar Yangon. This document is executed in two (2) original copies, one for each party.
