Terms and Conditions

This Purchase Order is issued by Accra Health & Rehabilitation Centre Ltd. and constitutes a binding agreement upon acceptance by the vendor. All services and goods described herein shall be delivered to the designated facility in Ghana Accra unless otherwise specified in writing.
The Physiotherapist engaged under this Purchase Order must hold a valid practising licence issued by the Ghana Physiotherapy Association and the Health Professions Regulatory Council of Ghana. Proof of licensure and professional indemnity insurance (minimum GHS 500,000 coverage) must be submitted prior to commencement of duties.
Payment terms: 30% advance payment upon signing of this Purchase Order; 40% upon successful delivery and installation of all physiotherapy equipment at the Ghana Accra site; 30% upon completion of the twelve-month service period, subject to satisfactory performance review.
All payments shall be made via bank transfer to the vendor's designated account at Ecobank Ghana, Accra Branch. Invoices must reference this Purchase Order number (PO-2025-0487) and include the vendor's Ghana TIN for tax compliance.
The vendor warrants that all physiotherapy equipment supplied is new, of professional grade, and complies with the standards set by the Ghana Standards Authority. A minimum two-year manufacturer warranty applies to all capital equipment.
Delivery and installation of all equipment must be completed no later than 20 July 2025 at the Ghana Accra facility. The vendor is responsible for all logistics, customs clearance (if applicable), and on-site setup within the Greater Accra Region.
The Physiotherapist shall adhere to the clinical protocols, patient confidentiality policies (in line with the Data Protection Act, 2012 – Act 843 of Ghana), and institutional guidelines of Accra Health & Rehabilitation Centre Ltd. throughout the engagement period.
Either party may terminate this Purchase Order with thirty (30) days written notice. In the event of termination, the vendor shall be compensated for services rendered and equipment delivered up to the date of termination, less any applicable penalties for incomplete obligations.
Disputes arising from this Purchase Order shall be resolved through mediation in Ghana Accra, in accordance with the laws of the Republic of Ghana. The courts of competent jurisdiction in Accra shall have exclusive jurisdiction.
This Purchase Order supersedes all prior quotations, verbal agreements, or correspondence between the parties regarding the procurement of Physiotherapist services and related equipment for the Ghana Accra facility.

NOTE: This Purchase Order has been reviewed and approved by the Procurement Committee of Accra Health & Rehabilitation Centre Ltd. in accordance with the Ghana Public Procurement Act, 2003 (Act 663) and the institution's internal procurement policy. The assigned Physiotherapist must complete a mandatory orientation and facility safety induction upon arrival at the Ghana Accra premises before commencing patient care. Authorisation & Acceptance
