1. This Purchase Order constitutes a binding agreement between the Purchasing Entity and the Physiotherapist service provider for the delivery of physiotherapy services within the emirate of Abu Dhabi, United Arab Emirates.
2. The Physiotherapist shall maintain a valid and current professional licence issued by the Department of Health – Abu Dhabi (DoH-AD) throughout the entire duration of this Purchase Order. Any lapse in licensure shall constitute immediate grounds for termination.
3. All physiotherapy treatments and rehabilitation programmes shall be conducted in strict compliance with the clinical guidelines, patient safety protocols, and regulatory requirements established by the Ministry of Health and Prevention (MOHAP) and the Department of Health – Abu Dhabi.
4. The Physiotherapist is required to submit a detailed treatment plan for each patient within 48 hours of the initial assessment. Progress reports shall be provided bi-weekly to the Purchasing Entity's medical director.
5. Payment shall be processed within thirty (30) calendar days from the date of invoice receipt. Invoices must include the VAT registration number of the Physiotherapist and reference this Purchase Order number (AD-PT-2025-04782).
6. All services shall be rendered at the facility located on Corniche Road, Al Reem Island, Abu Dhabi, United Arab Emirates, unless otherwise agreed in writing by both parties.
7. The Physiotherapist shall maintain professional indemnity insurance with a minimum coverage of AED 2,000,000, valid throughout the United Arab Emirates, and shall provide a certificate of insurance upon request.
8. Confidentiality of all patient records and medical data shall be maintained in accordance with the UAE Federal Decree-Law No. 45 of 2021 on the Protection of Personal Data and applicable health privacy regulations in Abu Dhabi.
9. Any modification, amendment, or cancellation of this Purchase Order must be executed in writing and signed by authorised representatives of both parties. Verbal agreements shall not be binding.
10. In the event of a dispute arising from this Purchase Order, the parties shall first attempt resolution through amicable negotiation. Failing resolution within thirty (30) days, the matter shall be referred to the competent courts in Abu Dhabi, United Arab Emirates, and governed by the laws of the United Arab Emirates.
11. The Physiotherapist shall not subcontract or delegate any portion of the services outlined in this Purchase Order without prior written consent from the Purchasing Entity.
12. This Purchase Order is valid for a period of six (6) months from the date of issue, covering the period from 1 July 2025 to 31 December 2025, unless extended by mutual written agreement.
