This Purchase Order is issued by the Islamabad Regional Health Authority to formally engage a qualified Psychiatrist for the provision of specialized mental health services within the jurisdiction of Pakistan Islamabad. The services described herein are subject to the terms, conditions, and regulatory requirements set forth in this document and the applicable laws of the Islamic Republic of Pakistan.

The Psychiatrist engaged under this Purchase Order must satisfy the following mandatory qualifications and licensing criteria to practice within Pakistan Islamabad:
- Hold a valid MBBS degree from a recognized medical university in Pakistan or an equivalent foreign qualification recognized by the Pakistan Medical and Dental Council (PMDC).
- Hold a valid Fellowship of the College of Physicians and Surgeons Pakistan (FCPS) in Psychiatry or an equivalent postgraduate qualification in Psychiatry.
- Be registered in good standing with the Pakistan Medical Commission (PMC), formerly PMDC, with a valid annual license to practice in the Islamabad Capital Territory.
- Hold a valid license to practice medicine issued by the Islamabad Capital Territory Health Department, Pakistan Islamabad.
- Have a minimum of five (5) years of post-fellowship clinical experience in adult and/or child and adolescent psychiatry.
- Be in compliance with all ethical and professional conduct standards as defined by the Pakistan Medical Commission Code of Ethics.
- Carry valid professional indemnity insurance with a minimum coverage of PKR 50,000,000, as required for medical practitioners operating in Pakistan Islamabad.
- Term of Engagement: This Purchase Order is valid for a period of six (6) months from the date of acceptance, commencing on 01 July 2025 and concluding on 31 December 2025. Renewal shall be subject to mutual written agreement and a new Purchase Order.
- Payment Terms: Payment shall be made on a monthly basis within thirty (30) days of receipt of a valid invoice from the Psychiatrist or the Capital Mind & Behavioral Health Clinic. Payment shall be processed via bank transfer to the account designated in the invoice. All payments are subject to applicable withholding tax deductions as mandated by the Federal Board of Revenue (FBR), Pakistan Islamabad.
- Working Hours and Availability: The Psychiatrist shall be available at the IRHA facility, Pakistan Islamabad, from Monday to Saturday, 09:00 to 17:00, for outpatient and inpatient services. On-call emergency availability shall be arranged on a rotating schedule as detailed in Item 03 of this Purchase Order.
- Confidentiality and Data Protection: The Psychiatrist shall maintain strict confidentiality of all patient records and medical information in accordance with the Pakistan Islamabad medical confidentiality standards and the applicable data protection regulations. No patient information shall be disclosed to any third party without written consent, except where required by law.
- Compliance with Local Regulations: All psychiatric services rendered under this Purchase Order shall comply with the Mental Health (Ordinance) Act, 2001, as applicable in Pakistan Islamabad, and all guidelines issued by the Islamabad Capital Territory Health Department.
- Termination: Either party may terminate this Purchase Order with thirty (30) days written notice. The Buyer may terminate immediately in the event of material breach, loss of medical license, or professional misconduct by the Psychiatrist.
- Dispute Resolution: Any disputes arising from this Purchase Order shall be resolved through amicable negotiation. Failing resolution within thirty (30) days, the matter shall be referred to arbitration in accordance with the Arbitration Act, 1940, with the seat of arbitration in Pakistan Islamabad.
- Governing Law: This Purchase Order shall be governed by and construed in accordance with the laws of the Islamic Republic of Pakistan, with specific reference to the regulations applicable in the Islamabad Capital Territory.
All services under this Purchase Order shall be rendered at the Islamabad Regional Health Authority facility located in Sector F-10/2, Jinnah Avenue, Pakistan Islamabad, and at such other designated locations within the Islamabad Capital Territory as may be mutually agreed upon in writing. The Psychiatrist shall report to the IRHA facility at the commencement of each scheduled shift and shall maintain a complete and accurate medical record for every patient seen.
This Purchase Order is issued under the authority of the Islamabad Regional Health Authority, Pakistan Islamabad, and constitutes a binding commitment upon acceptance by the service provider. By signing below, both parties acknowledge that they have read, understood, and agree to all terms and conditions set forth in this document.
