This Purchase Order is issued by Al Noor Medical Group LLC to formally engage the services of a qualified Psychiatrist for the provision of comprehensive psychiatric care, consultation, and therapeutic services at our primary clinic located in Qatar Doha. The Psychiatrist engaged under this Purchase Order shall be responsible for the diagnosis, treatment, and ongoing management of patients presenting with mental health conditions, including but not limited to major depressive disorder, generalized anxiety disorder, bipolar disorder, post-traumatic stress disorder, obsessive-compulsive disorder, and substance use disorders.

The Psychiatrist shall provide a minimum of forty (40) clinical consultation hours per week at the Al Noor Medical Group facility in Qatar Doha, West Bay. All services rendered under this Purchase Order must comply with the regulations set forth by the Qatar Health Regulatory Authority (QHRA) and the Ministry of Public Health of the State of Qatar. The Psychiatrist is expected to maintain full professional licensure throughout the duration of this Purchase Order and to submit any required continuing medical education (CME) documentation upon request.

3.1 This Purchase Order constitutes a binding agreement between Al Noor Medical Group LLC and the named Psychiatrist for the provision of psychiatric services in Qatar Doha. All terms herein shall be governed by the laws of the State of Qatar.

3.2 The Psychiatrist shall maintain valid professional indemnity insurance with a minimum coverage of QAR 2,000,000 per claim for the entire duration of this Purchase Order. Proof of insurance shall be submitted prior to the commencement of services.

3.3 The Psychiatrist is required to maintain strict patient confidentiality in accordance with Qatar's data protection regulations and the ethical standards of the Qatar Medical and Dental Council. All patient records generated under this Purchase Order shall be stored in the Al Noor Medical Group electronic health record system located in Qatar Doha.

3.4 Failure to meet the minimum clinical hour requirements outlined in this Purchase Order shall result in a proportional reduction of the monthly service fee. Repeated non-compliance (three or more instances within a calendar quarter) shall constitute grounds for termination of this Purchase Order with thirty (30) days' written notice.

3.5 The Psychiatrist shall not subcontract or delegate any portion of the services described in this Purchase Order to any third party without prior written consent from Al Noor Medical Group LLC.

3.6 All invoices submitted under this Purchase Order must reference the Purchase Order number PO-2025-QD-04871 and shall be submitted electronically to the procurement department at Al Noor Medical Group, Qatar Doha, no later than the fifth (5th) business day of the following month.

All services under this Purchase Order shall be rendered at the Al Noor Medical Group primary care and mental health facility situated at Level 3, Doha Tower, West Bay, Doha, State of Qatar. The Psychiatrist shall report to the facility no later than 07:45 AM on all scheduled working days. The facility provides the Psychiatrist with a dedicated consultation room, access to the electronic medical records system, a secure pharmacy for controlled psychiatric medications, and administrative support staff. The Psychiatrist is expected to adhere to the dress code and professional conduct standards established by Al Noor Medical Group and the Qatar Health Regulatory Authority.

By signing below, both parties acknowledge and accept all terms, conditions, and service specifications outlined in this Purchase Order. This Purchase Order shall remain in effect for the stated twelve-month period commencing 01 July 2025 and may be renewed by mutual written agreement of both parties no later than thirty (30) days prior to the expiration date.
