- 4.1 This Purchase Order constitutes a formal request for psychiatric services to be rendered by the named Psychiatrist at the Zurich Psychiatric & Behavioral Health Center, located in Switzerland Zurich. All services shall comply with the Swiss Federal Act on Human Research and the Swiss Medical Association (FMH) ethical guidelines.
- 4.2 The Psychiatrist shall provide all services in accordance with the Swiss Federal Act on Medical Professions (Medizinalberufegesetz) and shall maintain full professional liability insurance as required by the cantonal regulations of Zurich.
- 4.3 Patient data and psychiatric records shall be handled in strict compliance with the Swiss Federal Act on Data Protection (DSG/FADP). No patient information shall be disclosed to MediCare Corporate Health Services AG beyond the scope of the written medical report specified in Item 5, and only with the explicit written consent of the patient.
- 4.4 Payment shall be made via bank transfer to the account specified by the Psychiatrist's practice within thirty (30) calendar days of receipt of a valid invoice referencing this Purchase Order number (PO-2025-ZRH-04782).
- 4.5 All services are to be delivered within the geographic area of Switzerland Zurich, specifically at the practice address listed in Section 2, unless otherwise agreed in writing by both parties.
- 4.6 Cancellation of scheduled psychiatric sessions must be communicated at least 48 hours in advance. Sessions cancelled with less than 48 hours' notice shall be billed at 50% of the session fee.
- 4.7 This Purchase Order is governed by the laws of the Swiss Confederation. Any disputes arising from this Purchase Order shall be subject to the exclusive jurisdiction of the courts of the Canton of Zurich, Switzerland.
- 4.8 The Psychiatrist warrants that all services are performed by a duly licensed and registered medical professional holding a valid Swiss medical license and that all psychotropic medications prescribed are in accordance with Swiss pharmaceutical regulations (Swissmedic).
This Purchase Order is issued under the MediCare Corporate Employee Mental Health Support Program for the 2025 fiscal year. The Psychiatrist services outlined herein are intended to support the psychological well-being of up to twelve (12) designated employees of MediCare Corporate Health Services AG. All initial consultations must be scheduled within the first two weeks of July 2025. The Psychiatrist is requested to coordinate with the company's occupational health physician, Dr. Thomas Berger, for any cases requiring multidisciplinary intervention. All documentation must be prepared in German or English, in accordance with the linguistic requirements of the Switzerland Zurich healthcare framework.
