This Purchase Order is issued by Harbourview Health & Wellness Group Ltd to formally engage a qualified Psychologist to deliver clinical and organisational psychology services within the New Zealand Auckland metropolitan region. The Psychologist shall provide evidence-based psychological assessment, therapeutic intervention, and workplace mental health consultancy in accordance with the Code of Ethics established by the New Zealand Psychological Society and the regulatory standards of the Health and Disability Commissioner of New Zealand.
The Psychologist engaged under this Purchase Order shall operate primarily from the supplier's registered practice in Auckland CBD and shall be available to travel to client sites throughout the Auckland region, including but not limited to North Shore, Manukau, Waitakere, and the Hutt Valley periphery, as required by the terms of this Purchase Order.

4.1 Payment Terms: Invoices shall be submitted monthly in arrears. Payment is due within 20 business days of receipt of a valid tax invoice. All payments shall be made via electronic funds transfer (EFT) to the bank account specified in Section 1. Late payments shall attract interest at the rate prescribed under the New Zealand Late Payment of Commercial Debts (Interest) Act 1993.
4.2 Professional Standards: The Psychologist shall maintain current registration and professional indemnity insurance (minimum cover of NZD 5,000,000) throughout the term of this Purchase Order. The Psychologist shall comply with all applicable New Zealand legislation, including the Health and Disability Services (Complaints) Act 1993, the Privacy Act 2020, and the Mental Health (Compulsory Assessment and Treatment) Act 1992.
4.3 Confidentiality and Data Protection: All client records, clinical notes, and personal health information shall be handled in strict accordance with the Privacy Act 2020 of New Zealand. The Psychologist shall not disclose any client information to Harbourview Health & Wellness Group Ltd beyond what is necessary for billing and service reporting purposes, unless required by law or with the client's written consent.
4.4 Service Delivery Location: Primary service delivery shall occur at the Psychologist's registered practice in Auckland CBD. Where on-site services are required at client premises within the New Zealand Auckland region, travel time exceeding 30 minutes shall be billed at the hourly supervision rate specified in Line Item 04.
4.5 Cancellation and Termination: Either party may terminate this Purchase Order with 60 days' written notice. In the event of termination, the Psychologist shall complete all in-progress clinical engagements and deliver outstanding reports within 30 days of the termination date.
4.6 Dispute Resolution: Any disputes arising under this Purchase Order shall first be referred to mediation under the New Zealand Disputes Tribunal. If unresolved, matters shall be adjudicated in the District Court of Auckland, New Zealand.
4.7 Governing Law: This Purchase Order shall be governed by and construed in accordance with the laws of New Zealand.
By signing below, both parties acknowledge and agree to the terms, conditions, and scope of services outlined in this Purchase Order. This document constitutes a binding agreement for the engagement of the Psychologist for the duration specified herein.
