Description of Services – Psychologist Engagement

This Purchase Order is issued by Meridian Health & Wellness Group Ltd. to engage the services of a qualified Psychologist, Dr. Adaeze Okafor, for the provision of comprehensive psychological assessment, counselling, and therapeutic intervention services to employees of the organization. The Psychologist shall render all professional services at the company's headquarters located in Nigeria Lagos, specifically at 14B Adeola Odeku Street, Victoria Island. The engagement is in full compliance with the regulations of the Counselling Psychologists and Counsellors Practitioners Board of Nigeria (CPN) and all applicable federal and state health regulations governing the practice of psychology within Nigeria Lagos.

Terms and Conditions

1. This Purchase Order constitutes a binding agreement between Meridian Health & Wellness Group Ltd. and the Psychologist, Dr. Adaeze Okafor, for the delivery of the professional services described herein. All services shall be performed within the jurisdiction of Nigeria Lagos, in accordance with the standards set by the Counselling Psychologists and Counsellors Practitioners Board of Nigeria.

2. The Psychologist warrants that she holds a valid and current professional licence to practise psychology in Nigeria Lagos and that all services rendered shall be conducted with the utmost professional integrity, confidentiality, and adherence to the ethical code of the Nigerian Psychological Association.

3. Payment of the total amount stated in this Purchase Order shall be made within thirty (30) calendar days from the date of receipt of a valid tax invoice from the Psychologist's practice. Payment shall be made via bank transfer to the account designated by the vendor.

4. All client records, assessment data, and therapeutic notes generated during the course of this engagement shall remain the confidential property of Meridian Health & Wellness Group Ltd. The Psychologist shall not disclose any information to third parties without written consent, except as required by Nigerian law.

5. The Psychologist shall provide a minimum of forty-eight (48) hours' written notice for any cancellation or rescheduling of sessions. Cancellations made within this window shall be subject to a 50% fee for the missed session.

6. This Purchase Order is governed by the laws of the Federal Republic of Nigeria. Any disputes arising from this engagement shall be resolved through arbitration in Nigeria Lagos in accordance with the Arbitration and Conciliation Act, Cap A18, Laws of the Federation of Nigeria 2004.

7. The Psychologist shall carry professional indemnity insurance with a minimum cover of NGN 50,000,000 for the duration of this engagement and shall provide a certificate of insurance upon request.

8. This Purchase Order may be amended only by mutual written agreement signed by both parties. No verbal modifications shall be binding.
