Description of Services – Radiologist EngagementThis Purchase Order is issued by Alexandria Medical Imaging Center to formally engage the services of a qualified Radiologist for the provision of diagnostic imaging interpretation, clinical consultation, and radiological reporting services at our facility located in Egypt Alexandria. The Radiologist shall perform all duties in accordance with the Egyptian Medical Syndicate regulations, the Ministry of Health and Population guidelines, and the professional standards established by the Egyptian Society of Radiology and Nuclear Medicine.

- This Purchase Order constitutes a binding agreement between Alexandria Medical Imaging Center and the named Radiologist for the provision of radiological diagnostic services in Egypt Alexandria. Both parties acknowledge and accept all terms herein upon signature.
- The Radiologist shall maintain a valid Egyptian medical license, active registration with the Egyptian Medical Syndicate, and current malpractice insurance coverage throughout the entire contract period. Any lapse in licensure shall result in immediate suspension of this Purchase Order.
- All services rendered under this Purchase Order shall be performed at the Alexandria Medical Imaging Center premises located in Egypt Alexandria, unless otherwise agreed in writing by both parties. The Radiologist shall comply with all hospital protocols, infection control measures, and patient confidentiality standards as mandated by Egyptian law.
- Payment for services shall be processed within thirty (30) calendar days from the date of invoice submission. Invoices must reference this Purchase Order number (PO-2025-EG-04782) and shall be submitted to the Accounts Department at Alexandria Medical Imaging Center, Egypt Alexandria.
- The Radiologist is obligated to deliver all radiological reports within twenty-four (24) hours of image acquisition for routine studies and within four (4) hours for urgent or emergency cases. Failure to meet these timelines shall be subject to a penalty of five percent (5%) of the affected service fee per occurrence.
- This Purchase Order may be terminated by either party with a written notice of thirty (30) days. In the event of termination, the Radiologist shall complete all pending reports and hand over all patient records to the facility in Egypt Alexandria before the final working day.
- All intellectual property, patient data, and clinical records generated during the engagement of the Radiologist shall remain the exclusive property of Alexandria Medical Imaging Center. The Radiologist shall not reproduce, publish, or share any patient information without explicit written consent in accordance with Egyptian data protection regulations.
- Any disputes arising from this Purchase Order shall be resolved through amicable negotiation. Should negotiation fail, the matter shall be referred to the competent courts in Egypt Alexandria under the jurisdiction of Egyptian law.
- The Radiologist shall attend a minimum of two (2) continuing medical education (CME) programs per year relevant to diagnostic radiology and shall provide certificates of completion to the facility administration in Egypt Alexandria.
- This Purchase Order is governed by the laws of the Arab Republic of Egypt. All communications related to this document shall be directed to the procurement office at Alexandria Medical Imaging Center, Egypt Alexandria.
By signing below, both parties confirm that this Purchase Order for Radiologist services in Egypt Alexandria has been reviewed, understood, and accepted in its entirety. This document represents the complete and final agreement between the parties regarding the scope of radiological services described herein.
