Terms and Conditions of This Purchase Order

This Purchase Order is issued by Cairo Medical Imaging Center, a licensed medical facility operating in Egypt Cairo, and constitutes a binding agreement for the professional services of the named Radiologist for the duration specified herein.
The Radiologist shall provide all diagnostic and interventional imaging services in strict compliance with the regulations set forth by the Egyptian Ministry of Health and Population, the Egyptian Medical Syndicate, and all applicable national and local health authority guidelines in Egypt Cairo.
All imaging reports produced by the Radiologist must be completed within four (4) hours for routine studies and within one (1) hour for emergency or STAT cases. Reports shall be entered into the center's Picture Archiving and Communication System (PACS) and made available to referring physicians in Egypt Cairo without delay.
The Radiologist is required to maintain valid professional liability insurance with a minimum coverage of EGP 5,000,000 per occurrence, naming Cairo Medical Imaging Center as an additional insured party. Proof of insurance must be submitted prior to the commencement of services under this Purchase Order.
Payment for services rendered under this Purchase Order shall be made via bank transfer to the Radiologist's designated account within thirty (30) calendar days of receipt of a valid invoice. Invoices must reference this Purchase Order number (PO-EGC-2025-04782) and include the applicable VAT registration details.
The Radiologist shall adhere to the center's confidentiality and patient data protection policies in full accordance with Egyptian Law No. 151 of 2020 on Personal Data Protection. All patient records and imaging data generated in Egypt Cairo shall be stored securely and shall not be disclosed to any third party without written patient consent.
Either party may terminate this Purchase Order with thirty (30) days' written notice. In the event of termination, the Radiologist shall be compensated for all services rendered up to the effective date of termination, and all outstanding invoices shall be settled in accordance with the payment terms stated above.
The Radiologist shall attend a minimum of two (2) continuing medical education (CME) workshops per year relevant to diagnostic and interventional radiology and shall provide certificates of attendance to the center's administration in Egypt Cairo.
This Purchase Order shall be governed by and construed in accordance with the laws of the Arab Republic of Egypt. Any disputes arising from this Purchase Order shall be resolved through the competent courts in Egypt Cairo.
All modifications or amendments to this Purchase Order must be made in writing and signed by both parties. Verbal agreements or informal communications shall not alter the terms of this document.

Important Note: This Purchase Order is valid for a period of twelve (12) months from the Required Service Start Date. The Radiologist is expected to be physically present at the Cairo Medical Imaging Center facility in Egypt Cairo for all scheduled shifts. Remote or tele-radiology services are not covered under this Purchase Order unless a separate written addendum is executed by both parties. Authorization and Acceptance Issued By (Buyer)
