This Purchase Order is issued by MediScan Diagnostic Centre Sdn. Bhd. (hereinafter referred to as the "Purchaser") to engage the services of Dr. Ahmad bin Rashid, a licensed and registered Radiologist (hereinafter referred to as the "Radiologist" or "Service Provider"), to provide comprehensive radiological diagnostic and interpretive services at the Purchaser's facility located in Malaysia Kuala Lumpur. The Radiologist shall be responsible for the operation, supervision, and clinical interpretation of all imaging modalities available at the centre, including but not limited to X-ray, Computed Tomography (CT), Magnetic Resonance Imaging (MRI), Ultrasound, and Mammography.

3.1 This Purchase Order is governed by the laws of Malaysia and shall be interpreted in accordance with the Contract Act 1950 of Malaysia. Any disputes arising from this Purchase Order shall be resolved through arbitration in Malaysia Kuala Lumpur in accordance with the rules of the Asian International Arbitration Centre (AIAC).

3.2 The Radiologist must hold a valid medical practitioner registration with the Malaysian Medical Council (MMC) and a valid specialist registration in Radiology. The Radiologist shall provide copies of all relevant licences, certifications, and professional indemnity insurance (minimum coverage of RM 2,000,000 per claim) prior to the commencement of services.

3.3 The Radiologist shall comply with all applicable regulations set by the Ministry of Health Malaysia, the Malaysian Nuclear Agency (Nuclear Safety Act 2004), and the Malaysian Society of Radiology guidelines for radiation safety and patient care.

3.4 All patient data and medical imaging records handled by the Radiologist at the Malaysia Kuala Lumpur facility shall be treated as strictly confidential in accordance with the Personal Data Protection Act 2010 (PDPA) of Malaysia. Any breach of patient confidentiality shall constitute grounds for immediate termination of this Purchase Order.

3.5 The Purchaser reserves the right to audit the Radiologist's work product, including a random sample of no fewer than 5% of all reports issued per quarter, to ensure diagnostic accuracy and compliance with established radiological reporting standards.

3.6 Payment shall be made via bank transfer to the account designated by the Radiologist within thirty (30) calendar days of receipt of a valid invoice. Late payments shall attract interest at the rate of 5% per annum as stipulated under the Late Payment of Outstanding Debts Act 2017 of Malaysia.

3.7 Either party may terminate this Purchase Order with thirty (30) days' written notice. In the event of termination for cause, the terminating party must provide a detailed written explanation of the breach.

All services under this Purchase Order shall be rendered at the MediScan Diagnostic Centre premises located at Level 12, Menara Sentral, Jalan Tun Razak, 50400 Malaysia Kuala Lumpur. The Radiologist is expected to be physically present at the Malaysia Kuala Lumpur facility during standard operating hours (Monday to Friday, 8:00 AM to 6:00 PM; Saturday, 8:00 AM to 1:00 PM) and to be available for on-call duties as specified in Line Item 2. Remote interpretation of urgent studies may be permitted with prior written approval from the Purchaser's Medical Director.

This Purchase Order becomes effective upon signature by both parties. The Radiologist acknowledges receipt of this Purchase Order and agrees to all terms, conditions, and service specifications outlined herein. By signing below, the Radiologist confirms that all professional qualifications, registrations, and insurance requirements for practising radiology in Malaysia Kuala Lumpur are current and valid.

This Purchase Order (PO-2025-00847) is the property of MediScan Diagnostic Centre Sdn. Bhd. and is intended solely for the use of the named Radiologist service provider in Malaysia Kuala Lumpur.
