This Purchase Order is issued by Auckland Regional Medical Services Ltd. to engage the services of a qualified Radiologist for the provision of diagnostic imaging interpretation, reporting, and related clinical services within the New Zealand Auckland metropolitan area. The Radiologist engaged under this Purchase Order shall provide all diagnostic imaging interpretation and reporting services at the Mitchell Diagnostic Imaging Centre located in Ponsonby, New Zealand Auckland, as well as at the Auckland Regional Medical Services satellite imaging facilities in Northcote and Manukau. The Radiologist is required to maintain current registration with the Medical Council of New Zealand, hold valid specialist certification in Radiology, and comply with all radiation safety regulations administered by the New Zealand Radiation Protection Authority.
All reports must be delivered within the turnaround times specified in the service-level agreement attached as Appendix A to this Purchase Order. For routine CT and MRI studies, a final report shall be available within 24 hours of image acquisition. Urgent and emergency studies shall be reported within 2 hours. The Radiologist shall also participate in weekly multidisciplinary team meetings held at the Auckland City Hospital and provide written summaries of findings relevant to patient management.
4.1 This Purchase Order is governed by the laws of New Zealand and shall be interpreted in accordance with the New Zealand Contract and Commercial Law Act 2014.
4.2 The Radiologist warrants that all services rendered under this Purchase Order shall be performed with the skill, care, and diligence expected of a reasonably competent Consultant Radiologist practising in New Zealand Auckland.
4.3 The Radiologist shall maintain adequate professional indemnity insurance with a minimum cover of NZD 5,000,000 per claim, and shall provide a certificate of insurance upon request.
4.4 All patient data and imaging records shall be handled in strict compliance with the New Zealand Privacy Act 2020 and the Health Information Privacy Code 1994. No patient information shall be transferred outside New Zealand without explicit written consent.
4.5 Payment shall be made within 30 days of receipt of a valid tax invoice. Late payments shall attract interest at the rate prescribed under the New Zealand Interest on Late Payments of Government Debts Act 1983.
4.6 Either party may terminate this Purchase Order with 60 days written notice. In the event of termination, all services rendered up to the termination date shall be invoiced and paid in full.
4.7 The Radiologist shall comply with all applicable New Zealand Auckland regional health authority guidelines, including those issued by Te Whatu Ora (Health New Zealand) and the Auckland District Health Board successor entities.
This Purchase Order is valid for acceptance within 14 calendar days of the date of issue. The Radiologist shall confirm acceptance by signing and returning a copy of this document to the procurement office of Auckland Regional Medical Services Ltd. Failure to return a signed copy within the stipulated period shall render this Purchase Order null and void.

All correspondence regarding this Purchase Order shall be directed to the procurement office at the address stated in the header. The Radiologist is reminded that this engagement is subject to the New Zealand Auckland regional workforce planning framework and may be subject to annual review by the Auckland Regional Medical Services clinical governance committee. Any amendments to the scope of services, pricing, or delivery timelines must be documented in a formal amendment to this Purchase Order and signed by both parties before taking effect.
This Purchase Order does not constitute an employment relationship. The Radiologist is engaged as an independent contractor and is responsible for all personal income tax obligations under the Inland Revenue Department of New Zealand.
