This Purchase Order is issued by Colombo Diagnostic & Imaging Centre (Pvt) Ltd. to engage the services of a qualified Radiologist for the provision of diagnostic imaging interpretation, clinical consultation, and radiological reporting services. The engagement is specifically tailored to the operational requirements of our facility located in Sri Lanka Colombo, serving the broader Western Province patient population. The Radiologist engaged under this Purchase Order shall be responsible for the interpretation of all imaging modalities including but not limited to X-ray, Computed Tomography (CT), Magnetic Resonance Imaging (MRI), Ultrasound, and Fluoroscopy studies conducted at our premises in Colombo.

- This Purchase Order constitutes a binding agreement between the Purchasing Entity and the Radiologist service provider for the duration of twelve (12) months commencing 01 July 2025 and concluding 30 June 2026, unless terminated earlier in accordance with Clause 8.
- The Radiologist shall maintain valid registration with the Medical Council of Sri Lanka and shall provide proof of professional indemnity insurance with a minimum cover of LKR 50,000,000 for the entire contract period. All services shall be rendered in compliance with the regulations of the Sri Lanka Medical Council and the relevant health authority guidelines applicable in Sri Lanka Colombo.
- Payment shall be made within thirty (30) days of receipt of a valid tax invoice from the Radiologist. Invoices must reference this Purchase Order number (PO/SLC/2025/04782) and shall be submitted to the Accounts Department at Colombo Diagnostic & Imaging Centre, Galle Road, Colombo 03, Sri Lanka.
- The Radiologist is required to be physically present at the Colombo facility for a minimum of five (5) working days per week, between the hours of 08:00 and 17:00, with the exception of scheduled leave approved in writing by the Medical Director.
- All radiological reports generated under this Purchase Order shall be completed within four (4) hours for routine studies and within one (1) hour for emergency/urgent studies. Reports shall be entered into the facility's Picture Archiving and Communication System (PACS) and made available to the referring physician.
- The Radiologist shall comply with all data protection and patient confidentiality requirements as stipulated under the Sri Lankan Personal Data Protection Act and the ethical codes of the Sri Lanka Medical Council. Patient records generated in Sri Lanka Colombo shall not be transferred outside the country without explicit written consent.
- Any amendments, variations, or extensions to this Purchase Order must be documented in a written addendum signed by both parties. Verbal agreements shall not be enforceable.
- Either party may terminate this Purchase Order with thirty (30) days' written notice. In the event of termination, the Radiologist shall complete all pending reports and hand over all patient records to the facility within seven (7) days of the termination date.
- Disputes arising from this Purchase Order shall be resolved through mediation in Colombo, Sri Lanka, and failing resolution within sixty (60) days, shall be referred to the courts of competent jurisdiction in Colombo, Sri Lanka.
- This Purchase Order is governed by the laws of the Democratic Socialist Republic of Sri Lanka. All references to Sri Lanka Colombo in this document refer to the city of Colombo and its immediate metropolitan area within the Western Province.
