1 Full-time School Counselor services for the academic year 2025–2026. The School Counselor shall provide individual and group counseling sessions for students aged 6 to 18, covering academic guidance, career planning, emotional wellness, conflict resolution, and crisis intervention. Services to be rendered on-site at the Philippines Manila campus, Monday through Friday, 7:30 AM to 5:00 PM. 10 months (July 2025 – April 2026) PHP 45,000.00 / month PHP 450,000.00
2 Development and implementation of a comprehensive School Counseling Program aligned with DepEd Order No. 42, s. 2012 (Guidance and Counseling Program). Includes curriculum mapping, assessment tool procurement, and quarterly program evaluation reports submitted to the School Administration in Philippines Manila. 10 months PHP 15,000.00 / month PHP 150,000.00
3 Parent-Teacher-Counselor conferences (minimum 4 per academic year). The School Counselor shall facilitate structured sessions involving parents, guardians, and students to discuss academic progress, behavioral concerns, and developmental milestones. All conferences to be held at the Philippines Manila campus facility. 4 sessions PHP 12,000.00 / session PHP 48,000.00
4 Professional development workshops for teaching staff on student mental health awareness, trauma-informed classroom practices, and early identification of at-risk students. Minimum of 6 workshops per academic year conducted at the Philippines Manila campus. 6 workshops PHP 8,000.00 / workshop PHP 48,000.00
5 Emergency and crisis response services. The School Counselor shall be available for after-hours consultation (up to 10 hours per month) for urgent student welfare matters, including but not limited to bullying incidents, family crises, and mental health emergencies. Coordination with local hospitals and mental health facilities in the Philippines Manila area. 10 months PHP 5,000.00 / month PHP 50,000.00
6 Annual student assessment and progress reporting. The School Counselor shall administer standardized assessments, maintain confidential student records in compliance with the Philippine Data Privacy Act of 2012 (RA 10173), and submit comprehensive annual reports to the School Board of Directors. 10 months PHP 10,000.00 / month PHP 100,000.00

TERMS AND CONDITIONS
  1. Scope of Engagement: This Purchase Order authorizes the engagement of the named School Counselor to render professional guidance and counseling services exclusively at the Manila Integrated Academic Institute campus located in the Philippines Manila area. The School Counselor shall adhere to the Code of Ethics for Professional Counselors as established by the Professional Regulation Commission (PRC) of the Philippines.
  2. Compliance with Philippine Law: The School Counselor shall comply with all applicable Philippine laws, including but not limited to the Data Privacy Act of 2012 (RA 10173), the Anti-Bullying Act of 2013 (RA 10627), the Child Protection and Welfare Act (PD 603), and all Department of Education (DepEd) circulars and memoranda governing student counseling services in the Philippines Manila region.
  3. Confidentiality: All student records, counseling session notes, and personal information shall be treated with the highest degree of confidentiality. The School Counselor shall not disclose any student information to third parties without written parental consent, except where mandated by Philippine law or in cases of imminent danger to the student or others.
  4. Payment Schedule: Payment shall be made in monthly installments via bank transfer to the account designated by the School Counselor. Invoices shall be submitted by the 5th of each month for the preceding month's services. Payment is due within thirty (30) calendar days from the date of invoice receipt by the Accounts Payable department in Philippines Manila.
  5. Performance Standards: The School Counselor shall maintain a minimum of 90% attendance at the Philippines Manila campus. Failure to meet this standard for two consecutive months shall constitute grounds for termination of this Purchase Order with thirty (30) days written notice.
  6. Insurance and Liability: The School Counselor shall maintain professional liability insurance with a minimum coverage of PHP 5,000,000.00 throughout the duration of this Purchase Order. Proof of insurance shall be submitted prior to the commencement of services.
  7. Termination: Either party may terminate this Purchase Order with thirty (30) days written notice. In the event of termination, the School Counselor shall complete all ongoing counseling sessions and provide a comprehensive handover report to the School Administration within fifteen (15) days of the termination date.
  8. Dispute Resolution: Any disputes arising from this Purchase Order shall be resolved through mediation in accordance with the Alternative Dispute Resolution Act of 2004 (RA 9285) of the Philippines. If mediation fails, disputes shall be submitted to the courts of competent jurisdiction in the Philippines Manila area.
  9. Force Majeure: Neither party shall be liable for failure to perform obligations due to events beyond reasonable control, including but not limited to natural disasters, government-issued quarantine orders, or civil unrest affecting the Philippines Manila area.
NOTE: This Purchase Order is issued in accordance with the procurement policies of Manila Integrated Academic Institute and the guidelines set forth by the Department of Education for the engagement of School Counselor services in private educational institutions within the Philippines Manila region. The School Counselor named herein must hold a valid PRC license and shall renew the same prior to the expiration date. All services rendered under this Purchase Order are subject to the supervision and oversight of the School Principal and the Director of Student Affairs. Prepared By:
