This Purchase Order is issued by Brisbane Community Health & Welfare Services Ltd (hereinafter referred to as the "Buyer") to formally engage the services of a qualified Social Worker, Ms. Eleanor J. Whitfield (hereinafter referred to as the "Supplier"), to deliver community-based social work interventions, case management, and family support services within the greater Brisbane metropolitan area, Queensland, Australia. This Purchase Order constitutes a binding agreement for the procurement of professional Social Worker services in accordance with the Australian Social Work Council (ASWC) Code of Ethics and the Queensland Health and Community Services Act 2003.
The Social Worker engaged under this Purchase Order shall provide the following services to the Buyer's client base across Brisbane, including but not limited to the suburbs of Fortitude Valley, South Brisbane, West End, and the Brisbane River corridor:
- Individual and family counselling sessions (minimum 120 hours per quarter)
- Case management for vulnerable adults and at-risk youth in the Brisbane metropolitan region
- Coordination of multidisciplinary referrals to Queensland Health, the Department of Child Safety, and local NDIS providers
- Development and delivery of community resilience workshops in partnership with Brisbane City Council
- Advocacy and liaison with government agencies on behalf of clients residing in the Brisbane postcode areas 4000–4179
- Monthly progress reporting to the Buyer's Clinical Governance Committee

Payment for the Social Worker services outlined in this Purchase Order shall be made in three equal monthly instalments of AUD $21,417.00 (inclusive of GST), due on the last business day of each calendar month within the contract period (July 2025, August 2025, and September 2025). Payment shall be processed via electronic funds transfer (EFT) to the Supplier's nominated Australian bank account. Invoices must reference this Purchase Order number (PO-BNE-2025-04782) and be submitted to the Buyer's Accounts Payable department no later than the 25th of each month. Late payments shall attract interest at the rate prescribed under the Queensland Interest on Late Payments of Government Debts Act 2003.
- Qualification and Registration: The Social Worker must maintain an active registration with the Australian Association of Social Workers (AASW) and hold a current Working with Children Check (WWCC) issued by the Queensland Police Service for the duration of this Purchase Order.
- Professional Standards: All services delivered by the Social Worker shall comply with the ASWC Code of Ethics, the National Professional Standards for Social Workers, and applicable Queensland legislation, including the Child Protection Act 1999 (Qld) and the Mental Health Act 2016 (Qld).
- Confidentiality and Privacy: The Social Worker shall adhere strictly to the Privacy Act 1988 (Cth) and the Australian Privacy Principles when handling client information within the Brisbane service area.
- Insurance: The Social Worker shall maintain professional indemnity insurance with a minimum cover of AUD $5,000,000 and public liability insurance of AUD $10,000,000, with the Buyer named as an additional insured party.
- Termination: Either party may terminate this Purchase Order with 30 days' written notice. The Buyer may terminate immediately for material breach, loss of AASW registration, or failure to maintain a valid WWCC.
- Dispute Resolution: Any disputes arising under this Purchase Order shall be resolved in accordance with the Queensland Civil and Administrative Tribunal (QCAT) procedures or, where applicable, the Australian Consumer Law.
- Governing Law: This Purchase Order shall be governed by and construed in accordance with the laws of the State of Queensland, Australia.
- Work Location: Primary service delivery shall occur at the Buyer's Brisbane CBD office (400 George Street) and at community venues across the Brisbane metropolitan area as directed by the Buyer's Clinical Director.
By signing below, both parties acknowledge and agree to the terms, conditions, and scope of services set out in this Purchase Order for the engagement of the Social Worker in Brisbane, Australia. This document is valid for a period of three (3) months from the date of issue and may be renewed by mutual written agreement.
